Wow, On a break and all hell breaks loose. Where to start, going to make some nursing suggestions
Gail, you know somethings way off, but getting someone to listen is difficult. If your PCP is not responsive, You can do some of the following. There will be those that say I'm abusing the system, but girl until proven otherwise your symptoms say your bodies in trouble--You describe multiple major s/s's(signs/symptoms's), that when put together could lead to very serious consequenses if not addressed. NAUSEA, VOMITING, DIARRHEA, NEAR PASSING OUT, WEAKNESS. May have missed some
What they could mean: Diagnostically the list is too long, thats why intervention for correct diagnosis is so important.
How to get seen quickly: Call EMS, when they respond --do not minimize any of your symptoms. Be dramatic about each. Sounds wacky, but most people will be in denial or minimize s/s's till they bottom out. Make them transport you. That will get you inside the ER and on a stretcher. Once inside the ER you will be seen sooner than if you arrive by car and walk in or wheelchair from car.
Why are you so much at risk even w/o having a dx? Vomiting and diarrhea can lead fairly quickly to electrolyte problems. Electrolyte problems can lead to cardiac problems and other things like central nervous sytem problems(dysfunction). Feelings of passing out and symptom being made worse by positional(going from laying to sittting to standing) change in the presense of all your other s/s's must be assumed UNTIL PROVEN OTHERWISE that you are VOLUME DEPLETED. V&D causes a fluid shift from the vascular space to the gastrointestinal space, thus depleting the vascular volume. Vascular Volume Depletion can lead to shock, in this case it would be known as hypovolemic shock. But other things need to be ruled out like sepsis. Sepsis means infection. When infection is local only local s/s's exist. When infection goes systemic you have s/s 's body wide. Fever with your s/s's makes the dx easier. BUT without fever you could still be septic.
What tests should be done in ER: Broke them into separate lines so you can make a list to take with you to ER, so, you know if everything that needs to be done is being done.
Complete blood count-CBC Includes too much to describe each, but will show what's happening to all necessary components;
CMP-complete metabolic profile which includes all electrolytes except magnesium; and liver and kidney function test;
Blood cultures (to determine sepsis);
urinalysis shows many things besides urinary track infection; can indicate how kidneys are functioning particularly when CBC is available for comparison. If bacteria present a culture and sensitivity will automatically be done.
stool culture and c-diff toxin test (now includes a blood test done at same time as stool collection). The C-diff test is a completely separate test from a stool culture, make sure it's tested.
orthostatic vital signs--BP and pulse done laying, then sitting, then standing. Likely will be done first thing.
Other diagnostic tests that may be ordered chest xray, abdominal CAT SCAN. CT scan would likely not be ordered until labs are back.
Minimal initial treatment: IV normal saline, might be held until labs are drawn and orthostatic vitalsigns are done b/c if volume is corrected before the lab tests are done, it could alter test values. But if initial BP done by EMS responders is below a certain level, normal saline will be infused right away at a specified challenge amount. (definition left out).
Please Gail consider the above
People worry when Ems is called that they will have a big bill to pay for transport. Two considerations. A municipal EMS that transported, may bill your insurance and then send you a bill to pay the left over, Just call the EMS billing office and tell them there is no money to pay bill. Keep record incase you get a 2nd bill. Second consideration: Primary Municipal EMS may try to send you by private ambulance. This is more of a problem. Don't allow it, and for sure don't sign anything that says you will pay the bill.
Think that covers it, anyone seeing something I missed, please jump in.

((((((((((((((((Fuzzy))))))))))) thinking of you and your Mom sending you both strength

Fuzzy, in your pocket on all counts, hope MOM's okay.
Alyson missed saying congratulations 6th cancerversary when I saw announcement on OMG.
GMA and all having uncontrolled pain. 2009 was my escallating and uncontrolled pain year. Met my new PCP on DEC9th 2009. She was the first one too take it all seriously. In one week she had me in a controlled state. I was at a signifcant disadvantage from pre-op BMX till the time I met her. My previous PCP left after my last contact and then died of his brain tumor-- very sad. My disadvantage is I didn't have anyone that knew me. New PCP took what should have been a less than ten minute contact and didn't leave me for an 1 1/2 hrs, until all avenues of sources of pain were discussed , a plan, and scripts. Pain sources: Post polio that cresendoed into absolute worst form, fibromyalgia dx'd, chronic neck pain from trauma---so basically the musculoskeleletal system was screaming with every movement and the deep bone pain was as if I was on a constant dose of neulasta. Hope that all is descriptive enough b/c her treatment plan lead back too reasonable normal accepted level of pain--5-6/10 lifelong. That level was able to be tolerated without meds. But was 10/10(100/10) at this time. Plan: Fentanyl patch 25mcg, oxycodone 5mg (plain, no tylenol or NSAIDS, for me as they cause BP problems)-one to two for breakthrough pain, Ativan 1-2mg for sleep(horrific insomnia), Savella/milnapricin SSNRI for fibromyalgia and depression, Valium 10 mg as necessary for TMJ and neck spasms(only drug that worked for either).The Fentanyl was the biggest controller:)))). Fentanyl has primary pain control activity, but also affects serotonin levels a fact that few medical professionals would be aware off. Also, Savella over time kicked in with it's magic. The next part of the plan was to have me tested for food sensitivities/allergies. This turned out to be major. She went through MRT testing Laboratories, but results weren't in for 3-4 weeks. Then close to the end of Jan. I found Genelex/Genemdrx, drug interaction program. Identified that AI's were interacting with my antihypertensive Norvasc.
How all the meds have been used has changed over time as my body has healed is too long to write here. Fentanyl now only used for flairs that progressively decreased over time versus constant use until end of Aug 2010. Savella will likely be on lifelong. Valium rare, b/c spasming occurrs in response to other body pain. Insomnia progressively improved as pain was controlled and the stopping of AI's in SEPT 2011. Treatment for that for quite sometime had been 4mg of ativan plus melatonin 10 mg. That's high for both, but sleep was imperative to healing. Now each drug is as necessary. I start now with 2.5 to 5 mg of melatonin and MAY BE 0.5 mg of ativan. Know that this is lengthy, For me the resolution of uncontrolled pain started the day I met Saint PCP.

edited to add - Sassy! hey!
Hi all - I wrote a post two nights ago but I see now that it did not go through due to an ice storm here. The connection was lost and I figured as much. Will try to rewrite though being brief since memory is short.
DUNES - You know you are needed, and it gets rough but here we are anyway. RIDER, LILY, GRANNY, GAIL, CINDY, feeling for all of you. Hugs.
The bankruptcy topic - we filed after I was so ill and we had to get away from the outside wood boiler next door. It was a matter of living elsewhere or dying there. Yesterday from the oncologist in Indy, btw, I found out the lung cancer had begun way back when I had the presurgery xrays, it was there. Financially, we had not one late payment before that. But to pay for the txs I needed was not an option with the high house/business barns payment. I gave up my vehicle too (no memory to get anywhere anyway back then) and we ened up renting here on 80 acres, I feel the hand of God was on this deal, I said I am here to heal or to die, it is so beautiful all around us. But the filing, it is a vague issue for us, we forged ahead, feeling that we had to do what we had to do.
GAIL - I certainly hope you went to ER, are comfortable and healing. FUZZY - will be thinking about you there at your mom's side. ESTHER - love the pictures and my favorite of all it The Old Madrone tree. I am a tree person and a hugger too, if I took pictures it would be of trees and their wisdom and life. You got me there, right in the heart, so lovely.
The news from the oncologist I was referred to by other oncologist was daunting news. I trust her, Dr. Kathy Miller, and defer to her tx plan. A double dose of estrogen rxs that complement one another. No chemo, rads, surgery can do a thing for this, that is the only choice still. She said the lung cancer is real, it is from the bc. I did not know. I was afraid it was coming, was undetected per say... I did not know it had been here since even before I found the mass. Just growing. Nothing changed except someone said, yes that is cancer in your lungs, without doubt. Took one who has seen it five thousand times to know it for what it was and call it. But it is ER + and PR+ by 95%> so this is the choice. She said this complementary therapy can shrink the cancer, even in the nodes. She is one of the best, a bc specialist with numerous trials under her belt, so I am glad to have this information and insight from her. Going to my in-area oncologist Monday for first injections.
Will check on everyone later.

Shiela I am so glad to hear you have new plan that works to keep you comfortable.
I did go ahead and go to the ER. Everyone was correct, I do have a concussion and more than that I seem to have normal pressure hydrocephalus. I have a referral to a Neorologist and I need to go ahead and make the appointment.

Now I am adding a pulmonary sleep doctor. It might take 7-10 days before someone calls me with appointment time. I'm happy and sad, both at the same time.

Rider-- take some pillows and punch them, as much as you need too. Scream too. But scream into the pillow..In 1995 Mom had a critical illness. Did the primal scream. Lost my voice for a week. Something(spirit) said scream into the pillow. Did and didn't loose my voice. Have used the pillow since. No clue why this made a difference, but it does

Our weather here in March - Amazing, living above the fog - Later on in the day when that fog rises, it comes to greet us. But later it was a beautiful sunny day.. Hello March!
