Multi-centric Breast Cancer
I read in my report :
"Findings in the right breast are highly suspicious of multicenteric disease extension and axillary lymph node metastasis for which biopsies were obtained."
Sound looks bad and I am scare too much . what is that ? how much worst is this? what is the meaning of multicenteric ?

I read a lot of subjects related to multicenteric disease :
the best sentence was "Another point of interest to both the patients and the doctors is that the diagnosis of a multicentric or a multifocal tumor does not affect the overall survival of the patient."
the worst things I found there is no treatment to this type of multiple tumor, no respond, and survive rate is bad.
I scare to much. I can not control my feeling . any help?

Hope99, I do not know what that means and probably many here also do not. You can google, or better, ask your doctor. I know you are concerned about the node status, but I had 2 positive nodes, no distant metastasis, am still Stage II, and living large 6 years later. None of us are guaranteed that we won't recur, but worrying and agonizing about what might happen won't change a thing, and just drive you crazy, lol (been there, done that). Try to relax, easier said than done, until you get input from your doctors.

Google Said :
"Multifocal and multicentric breast cancers have been correlated with poor prognostic factors and worse outcomes versus unifocal disease. We evaluated the impact of multifocal and multicentric disease versus case controls with unifocal disease, matching for age, grade, T-, and N-stage. A total of 110 patients with multifocal (n = 93) or multicentric (n = 17) disease and 263 matched case controls were identified with a median follow-up of 53 months and 64 months, respectively. The actuarial local control rates for the multifocal/multicentric and unifocal group were 88% and 97%, respectively at both 5 and 10 years (p < 0.001). On multivariate analysis, multifocal/multicentric disease remained associated with higher local recurrence after controlling for other covariates including surgery type. The disease-free survival rates in the multifocal/multicentric group at 5 and 10 years were 75% and 71%, respectively, versus 87% and 78% at 10 years (p = 0.01). On multivariate analysis, multifocal/multicentric disease was no longer associated with worse disease-free survival. There was no difference in the cohorts in terms of regional control, overall survival, or cancer specific survival. Our findings suggest that multifocal/multicentric disease may be associated with worse outcomes versus unifocal disease regardless of type of surgery. This suggests a more biologically aggressive cancer and may be an important consideration when managing these patients. Further studies are needed to better understand the impact of multifocal/multicentric breast cancers on outcomes."

Hope99,
I don't disagree with this study, but it's important to keep in mind, multi-anything is highly correlated to more advanced stages of cancer. So while yes, it has a poorer prognosis, it's the equivalent of saying Stage 3 is worse than Stage 2 which is worse than Stage 1. It's just another datapoint and I would not overly focus on it. There is another thread that shows multi-anything has not been indicated as an independent factor in a poorer prognosis.

I hope to find experiences that help us to understand this issues .
Multi-focal and Multi-centeric almost same.

multi focal means more than one area of cancer in the same quadrant of the breast. Multi centric means disease in different quadrants of the breast. This is my lay persons understanding
I had 9 areas of IDC,multi focal. Ranging in size from 2mm to 10mm. I also had a diseased node containing a 9mm growth
My oncologist told me that it is not a huge factor. They look at lots of factors such as grade, her2 status, nodes etc and multiple tumours doesn't necessarily mean a worse outcome.
I have read some of these studies as well including the one you cited above. Ive read ones that indicate multi focal / centric disease would indicate a higher risk of spread to nodes but not a poorer overall outcome. Speak to your doctor. Good luck!

Hello!
I understand your fear, as I had it too. I was DXd in January, 2009 at the age of 48. Had five foci in different quadrants, the largest of which was 2.3 cm. They were all different kinds. Did AC/T, some Tamoxifen, had LMS, and as of today, remain NED. Here's the thing, all you can do is hope for the best. Live your strongest life. Do it all. Eventually you will get to the day that you don't think about it as often as you did the day before. It takes a while, but it is possible to get there.

So grateful to read this. I’m terrified of the Multifocal/multicentric combo aspect of this. So good to read your post. I was just recently diagnosed a week ago. Bone scan tomorrow and terrified...

Hi mamamoose,
I have multi-centric. The other two small tumors only showed up on an MRI with contrast dye.

Mamamoose, studies have shown that prognosis of multi-focal or multi-centric disease is equivalent to the prognosis of the largest tumor, and that is what determines the stage. Multi-focal and multi-centric disease is usually more aggressive though, which is why prognosis looks worse compared to single tumor disease. I had multi-focal disease, I think I had one large tumor >3cm and maybe four smaller "satellite" tumors, plus a large area of "non-mass enhancement", and after chemo all the small tumors and area of non-mass enhancement were gone. Just a smattering of cells from the largest tumor remained on my surgical pathology. It's not a death sentence. Good luck!

similar diagnosis here. I had 17 multi foci in my L breast, R breast was standard IDC. Had pretty aggressive chemo and double mastectomy. My path results were clean. No evidence of disease. It’s not very common and information is sparse. Best of luck to you!

That’s excellent to hear! All of this is so overwhelming. It’s like let’s get this show on the road.
