Please Share Your Opinion? I'm Going to Reject Hormonal Therapy
Hello everyone!
I'm 27 year old breast cancer, from İstanbul. This forum was great support to me during my treatment, therefore I would like to open a subject regarding my decision and take your support.
My cancer type is Her positive (3+), ER positive (%7), PR negative. I have started with 4 rounds of AC/FEC (sorry I'm not sure if these are the correct names) then continued with weekly taxol and herceptin treatment which I will almost complete in 2-3 weeks. I'm also using lucrin which I totally hate! I will continue with radiotheraphy.
My onchologist just mentioned that beside of using herceptin for 1 year, he is thinking to give Tamoxifen to me for 3-5 year since I'm %7 ER positive. I'm so fed up with all these pills, hormonal effects, and it is only %7 percentage, I'm thinking about rejecting it.
What are your opinions? Anyone faced similar issue and give similar decision?
I'm also curious about the side effects of Tamoxifen, users could you let me know? Is there again menaupouse, hot flashes, no sex drive?
I'm newlywed and this treatment is taking control of my life I do not want to use any more extra medication for this kind of low percantage, please no one take me wrong.

I can relate to your concerns about tamoxifen, I had the same. I'm 32 and was diagnosed at 31 with triple positive IDC, went through chemo, surgery, radiation and then it was time to start tamoxifen. I wasn't keen on starting it but I had to give it a fair shot. I tried it and I was having some side effects that were manageable and others like anxiety that were not too manageable. Through discussions with my MO we agreed to cut the dose in half to see if that would help. Some side effects went away but the anxiety remained. My anxiety got better when I went off it.
I also tried Zoladex to shut down my ovaries, again, I had the anxiety come back. My MO is very much a quality of life kinda guy and through a really long discussion we decided that we would try to see if the anxiety would go away again without the Zoladex first before prescribing anti-anxiety meds. Given my particulars, he's left the decision up to me as to whether or not I continue with any form of hormonal therapy. I think my body is just very sensitive to hormonal fluctuations.
You are more likely to hear about people who have side effects on sites like this since that's what people are talking about but I have seen many women to take tamoxifen and are able to manage the side effects. Unfortunately it's one of those situations where you won't know the side effects unless you try.

I was older than you, 36 and while on Tamoxifen I only had hot flashes. You could always try it and see how you feel. Since you already are on Lucrin/lupron to shut down your ovaries and preserve them -Tamoxifen may not feel any different. But I totally understand that at 7% ER+ you may question the need for hormone therapy. But on the other hand there are many ER- women who would love to be in your shoes and to have an additional option to fight their cancer. Very tough situation.

That's a tough decision - with such a low ER% I can see why you might not want to take it, but a couple things to consider.
From what I've heard, typically, the younger and farther away from menopause you are, the fewer side effects you tend to get. I don't know how true that is, but I've heard it from multiple sources.
What my oncologist told me, is that the most common side effects, those he called “quality of life" side effects such as hot flashes, go away once you stop taking the drug, so it is always possible to give it a try for a few months. You might be one of the many people who have minimal if any side-effects. I have a friend who has been on it for 8 years with not a single side effect, and I am going into my 3rd year and have had only mild ones - nothing I can't live with. Incidentally, she is 5 years younger than me (I am 45). If you do find the side-effects more than you want to live with, then you could always stop the drug without ever having to wonder “what if I had tried it".
Tamoxifen does not cause menopause, by the way. It decreases the effect of estrogen in some tissues (like the breast) but enhances it in others like bones and the uterus. The most common side effect is hot flashes, but I have known a few women in real life who take it and have not experienced that. I myself have had mild ones - more of a minor annoyance than anything else.

Thank you so much for great answers! It really made me think, if my doctor insists maybe I will first try and see the side effects to me, maybe I could manage side effects. I will have my routine control this week and open this subject again.
It is only really hard to decide with that low ER percantage, of course I would not hesitate if my percantage was higher.
I will keep you posted about my decision, in the meantime I would glad to hear your experiences and comments.

For anyone who is thinking of not taking Tamoxifen because of the side effects, I would like to share this information that different manufacturers add different fillers to this medicine which causes different side effects. If you try Tamoxifen and you can not handle the side effects, look at your bottle and see who manufactured it. Then next month switch to another company and see if the side effects are less. Try it and then decide if you can handle it or not. This way you won't blame yourself in the future for not trying.

I am in the same situation, although a little older (38). My cancer is only 5% ER positive. So my MO suggested I try tamoxifen but said he wouldn't hesitate to take me off of it if the side effects are not manageable. I've decided that I might as well try. Although the benefits are small, I may be one of those people that experience no side effects. But, of course, everyone's decision will be a personal one.

I am struggling with the same thing only I am thinking about not taking herceptin and tomaxifan . I just at the end of the day think its a crap shoot as to if it comes back or not. I have had a horrible time on chemo's even the taxol every one claims is easy yet after AC then straight into taxol herceptin so 5 months of chemo my body is done . I jus dont trust anything anymore with what they say about odds and chances. Some of these ladies do it all and bam they are stage Iv . I have to decide quickly but I am leaning towards not doing it. I have yet to have one scan or test which makes no sense. My onc told me day one my tumor was small but very agressive and I had a 50/50 chance it has already spread. WTF ? But no pet scan or bone scan and I have had terrible hip and right leg , lower back pain for months. I so fed up that I dont trust the medical field anymore. So now I dont tell them anything cause nothing is ever related to chemo in there minds . Sorry to go on I was looking for someone questioning the same as me altho probally different reasons .

Our dx is very similar and I am only 4% er positive. So.....will get my tamoxifen rx in a week. Going to try it and see before I decide against it. I too am tired of all all this chit, and it is a crapshoot if it reurns, as someone's already said. But feel like giving it a try

DSW1976 - Not 'everyone' claims that Taxol is 'easy'. I, for one, found A/C easy and Taxol nasty. There are many of us here who found Taxol to be a lot worse than other Chemos. It does seem that if A/C is easy then Taxol will be nasty and visa versa (nasty A/C - easy Taxol). We are each unique so not all have the same experiences on anything. I did have 5 weeks between A/C and Taxol as I did A/C neoadjuvant with surgery 2 weeks after last A/C and started 12 weekly Taxol 3 weeks post surgery and then started rads a week after last Taxol. I started Femara a week after starting rads and have been on it since and will be on it 'forever'.
As I see it - IF I didn't do/hadn't done everything possible to fight the Monster, how could I face Hubby, Sons, and G-daughters IF it comes back and I had not done everything possible? To me, 'odds' mean nothing. It is either 0% or 100% as either I have a recurrence/metastis (100%) or I don't (0%). What happens to other individuals does not mean 'it' will happen to me or what I experience is what anyone else will experience.
We each have to make our own decisions. I will never regret doing everything I could as I am 5 1/2 yrs post DX with a type that only has a 25% - 40% that make it to 5 yrs - I'm still here and still NED so that's what matters to me and my family.

Believe me I am taking in eveything you all are saying. I have been on herceptin since jan 3 rd with taxol . I am going to alk with my onc Friday im just so lost and fed up. It is personal and at the same time I feel you all are the only ones that understand so I truely take any advice to heart . Are there any tests to see if chemo worked or if herceptin has worked? Thank you so much for taking the time to help me . Also what about scans whats bthe best to see if it has spread ? Currently I cant have an mri due to expanders. I kind of am scared to talk to my onc and his nurse p is not any easier. Im just so exhausted - bed ridden alot and I am going nuts. Im inmpain 24/7 I sleep 35 to 45 min streaks thats it . so sorry to go on with the poor me when so many women and children have it so much worse. I need help and guidance . Thanks again... AC was tough but taxol is the icing on this nasty cake for me and I am praying its no the herceptin .

DSW1976- Please do not feel bad about writing this thread, you had the similar questions in my mind. I totally understood how you feel. I just could recommend you keep going on with herceptin, since there are few side effects but the results are great. Please keep writing about your decision.
From my side, I had my doctor appointment yesterday, we talked about Tamoxifen and then he said that he is also thinking about 3 year Lucrin which totally drived me crazy. I know that as a doctor, he had to think about all possibilities but I began to feel that I'm just a machine who is producing cancer and they will use all these drugs to stop it. I'm only 27 and I rejected lucrin for 3 years, I do not want that long period menaupause. It is only 7% ER positive, I will think about tamoxifen but lucrin or zoladex is not acceptable for me. I'm really exhausted!
I do not feel that my doctor is not at all understanding that I'm a young patient and he is not understanding me. After finishing taxol, I will ask second opinions.
