Who has had implants removed then had a DIEP, GAP or PAP?
I had to have my implants removed on 2/13/14 after 4 infections. I never thought that the implants were uncomfortable while they were in, but now that they are out, I can't believe how good I feel! I really don't even mind not having breasts. All of the extra skin and tissue is a pain, but other than that nothing really bothers me.
I'm considering giving reconstruction one last try. Both my PS and my ID Dr. agree that I should not have implants again...they feel that my body did not like having the foreign objects in it. So the best tissue donor site...based on CT scan...is the back of my thighs. So I'm considering a PAP flap.
So, my question is: How different does a tissue transfer breast feel vs. an implant? Meaning how will it feel inside of my body. Can anyone explain it?

I have implants that are so uncomfortable and painful that I can't wait to get them out. I have surgery on June 4th to do a PAP Flap so would welcome any advice. Has anyone experienced relief after taking implants out and going with a tissue transfer procedure instead. Would like to speak to anyone that has done a PAP procedure before

Lfaith~ I posted this question on 2 other threads after no response here, and several ladies responded that the tissue transfer breasts felt wonderful. They feel like a natural breast. I also talked with a couple of women who had the PAP flap...I will try to remember their names and maybe you can send them a private message. One is named Catie2013 and I can't remember the other name...sorry.
There are a couple of active threads that talk about PAP flap procedures. This one is for NOLA http://community.breastcancer.org/forum/44/topic/736412 and this one is for Charleston, SC http://community.breastcancer.org/forum/44/topic/766866. Both are hospitals that are well known and highly regarded for their tissue transplant procedures.

thanks for all the information. I live on Long Island,NY and I am having a dr. Peter lorn who is located at 833northern blvd in great neck do the surgery, he is well known for this type of surgery. You can look him up online. I will keep you updated. I did decide to go ahead with it and I am scheduled for surgery on June 4th.

I read one of your past threads and you went through what I did, I do not have out of network benefits, so I am waiting to hear if I get approved, the surgeon believes he should be able to but I haven't heard yet.

Tamiami- If you go to the DIEP 2014 thread many of them have had implants removed and have done some kind of flaps. I know they will all jump in to answer your questions. I am sorry I cant I haven't had implants but I am having my BMX straight to Diep in May. The ladies tell me their breasts feel warm and just like the originals

I had an implant for a year and then went to a DEIP flap. The flap is sooooooo much better! My implant felt hard and the flap feel like a normal breast. I always felt like I had a rock on my chest with the implant. Good luck!

Tamiami, I had my implants removed and a TUG/PAP procedure done 2 years ago (TUG on the left and a PAP on the right). There's some pretty extensive threads on here from several years ago with ladies who had TUGs done. I've posted quite a bit about the two because, at the time, there were not many PAPs. I'll try to find the link for you, but feel free to PM me. I can speak with great authority on the difference between implants as well at the TUG/PAP options. The flap is sooooooo worth it!

Tamiami - I was just not happy with my implants..and was thinking of getting implant revision (have had the one side revised a few times) Dr suggested a flap..Due to previous abdominal surgery I was not a candidate for the usual tram/diep..so they told me I could do GAP or TUG..said there was more tissue for GAP but that they would do that over 2 surgeries..so I decided on TUG. My initial surgery was about 10 hrs..started around 8, was in recovery around 6pm..and in my room about 8 or so..by the next morning I was back in surgery as my flap had failed..a 3 hour surgery turned into 8 hrs as both went down while in surgery.. I was hoping for nicely shaped legs after and now my legs actually seem bigger..which makes no sense..but they rub now mid inner thigh and they never did before..my one leg is numb..they are really really sore when I walk (feel bruised all over) and I have lost alot of volume in one side due to staph infection..and still have an open wound.. I had my drains in for 7 weeks in my legs.. I do love the fact that I do not have any of those weird indents or movements from the implants and I do like the results on the one side..the other side is gonna need some help..I still have an open wound on the other side.. 3.5 months later and the Dr just put in a flexible drain a couple weeks ago (that drains into gauze to try to get the area to heal and close).. If I didnt have all the complications, I think I would have been happy..just wish I could have used abdomenial tissue

marial~ I'm so sorry that you are dealing with all of this! Sending you positive thoughts for your wound to close and that they are able to help with the breast you aren't satisfied with! Infections suck! Did you have your surgery locally?

yes, I did it locally..being on the west coast I didnt want to travel cross country for a surgery like that.I was in the hospital a week and went home basically not able to do anything on my own..so I would never have made it on a plane home..
I really hope when all is said and done I will be happy with the decision to do this..but we will see.if both sides looked like the right then I would be happy..my surgeons did say that they have never seen anyone with all the complications I have had..they do flaps almost daily and they have never seen what I have experienced..or even close to it!
i think I have decided to try to do the 3d tattoo once this part is done..cant imagine going through with the whole nips after all this..

It's been over a year since I had implant surgery and I feel worse now than I did right after surgery. My silicon implants are underneath my muscle. My right side is very painful. I work on a computer all day and it DOES use those muscles. I feel a lot worse on the days I work. I am also a swimmer and think I might have pulled my muscle doing that. I feel like I'm never going to heal unless I take more time off work. (or maybe I just won't heal)
If this keeps up, I will do the DIEP (or other similar) procedure. I'm feeling very frustrated!! I'm glad I did the mastecomy, but not sure about the implants.

Hi! I did the PAP last week. Recovery is going well. Even in the Boston area this is a newer surgery.. its important to get an experienced DIEP surgeon. I intend to post more as things progress; to help others. I just wanted to warn you that there is a member on this site that sent me a pm -without any posting between us -without any blogging-without any discussion etc-to show me very upsetting pictures and info about her unfortunate PAP experience. What she neglected to mention in her pm to me was she used an inexperienced surgeon. I learned this by clicking on her other posts to discussion groups. So I just wanted to warn you that she may do this to you. It really scared me- so I wanted to forewarn you. If this happens to you research what else she has posted. No one (good or bad) experience should determine our path. Please feel free to look at surgeries 2016 I intend to participate on that bloc. -

Hi Mel2328, I am having a SGAP in a couple months. I am having an implant removed. I got my mastectomy in the states but now live in switzerland. The implant is basically falling down and dragging my skin and scar with it. It is getting more painful. I cannot stand having the implant. I am in good hands here though. My doctor learned from the best flap PS in Zürich. Do you have drains at both sites? Is going to the bathroom hard? How much pain have you been in compared to the mx? Thanks!!

Mel2328 thanks for your comments about your PAP flap - I hope your recovery is going well! I would be very interested in updates as time goes on and any thoughts or advice you might have for someone who is considering it as well. I met with a PS today to discuss my reconstruction options. I won't have surgery for a few months (I am currently doing hormone therapy to try to shrink my tumor) so I have some time to research. Due to my build I am not a candidate for many of the other flap procedures. The PS is experienced with the PAP flap (as I read through the reconstruction thread I realize this is important). My other option is implant with fat grafting. Since the fat will most likely be from the thighs it seems like I should seriously consider the PAP. I am a runner but the PS says he has done the PAP flap on other distance runners who have been able to resume running after they recovered.

Hi Fe_princess. I also had implants that didn't work and like you; one sling broke so the implant hit my rib. I also got an infection in the other implant. I really wanted an SGAP but Bc I had a bilateral Dr preferred PAP Bc he can do PAP in both breast in one surgery. (He is able to keep you lying on your back with PAP) Whereas with SGAP he has to flip you and he wont do that more than once on a surgery. Are you having just one side done? Yes I had drains at both sites. (1 on each breast. 1 in back of each leg) they took them out after 2 weeks. The first week in hospital is a blur. So in my opinion it is easier than the mastectomy esp since they keep you so long. Some serious pain at first but it lets up fast. First week your hardly conscious. Second week you feel pretty good UNTIL the nerves in donor site wake up. That's painful for a week. I'm 3 weeks post op; so that's all I know so far. Go to DIEPS 2016. Ive posted more there and many similarities in recovery. Just a different donor location. Going to the bathroom is very hard Bc I can hardly sit but you should be ok I think. I did buy a handicap toilet and a soft vinyl cover seat that was helpful. Also there are girls go go sticks on amazon that allow you to pee standing up (marathon runners use them) that's been a life saver Bc I can still hardly sit! Check out other website and feel free to pm me. I'm happy with results. Legs look good; breast are small but Dr warned me about that. Ill do fat grafting. Talk soon!

Hi Lunderwood I'm doing well. Thank you for asking. See my other post I think it will help you. I work out a lot so had limited options also. Although I had a low BMI; I was a full C. Dr warned me with DIEP PAP alone I wouldn't be that size again. In fact with PAP and fat grafting ill be lucky to get a full B.
Bc I'm so active implants didn't work for me. See my other posts in DIEP. There are very few PAP people. I'm happy to answer anything or help in any way. Also; ask surgeon if you can talk to the runner that he did. Where do you live? Not many PAPS done here in Boston yet! Talk soon!! -
Is anyone else experiencing more and more feeling in DIEP flap? I have had this experience, right now I am getting almost a deep felt itch under my numb underarm area where lymph nodes taken. It feels so weird. Also stomach area gaining more feeling. I think this winter I will make an appointment with ps for fine fat grafting. The fat grafting I had before seems to have been absorbed. I like my DIEP overall.

Mel2328 thanks for the great feedback and for all the details - these help me with all the considerations and realities that the PS don't mention. I also appreciate your suggestions. My medical team is all part of University of Texas Southwestern Medical Center in Dallas, TX. I actually live about an hour and half away from Dallas but it is totally worth the drive for such great medical care and from what I can tell it's all very progressive. The tough part of living in a rural community is not having any local support groups or BC mentors to help me through this journey. This discussion board has definitely helped! I just wish I lived closer to a few of the gals and could actually meet up. Oh what was the time frame between your initial BMX and the PAP? Did you have expanders in between? Somehow I thought the expanders were only associated with implants but if I understood the PS correctly I will go through the expander process with either procedure. Oh and if I could also ask what was the length of hospital stay and return to work or expected return to work after each surgery. Unfortunately I changed jobs last year so have fairly limited vacation/PTO time so this too will have to be part of my decision process. Oh well I am just glad that I have a few options to consider.
