WisconsinRandi has posted about her full reconstruction, sharing that her photos are on Dr. Khouri's site. Hers are the top left photo(s). At the conference she and I both attended in October (was great fun meeting her!), I also met the patient whose photos are "Bilateral Mastectomies #3" on that same page. She was a young (early 40's maybe, if not younger) woman who I believe had D or larger cup-size breasts fully reconstructed.

mradf,I have never heard of signing a waiver saying you will not disclose your doctors name,is this a common practice?I hope that everyone on this forum would feel free to post about the good bad and the ugly about any doctor or hospital.If I had an excellent experience with a doctor or hospital I would want others to know,by the same token I would want to make others aware if the reverse was true.If this is something that is not allowed on this forum,it would be important to know.Also psf has posted about her reconstruction with fat grafting.I can't wait until I am posting about mine!

Yes! She is actually in the middle of a few steps to her final results and has posted about it both here and on my forum.

I have heard of some surgeons requiring waivers to be signed. Someone had told me that the surgeon I was considering in NY (before I learned that Dr. Khouri had pioneered this procedure) did. The day of my consult, my car's fuel line froze so I had to cancel. My consult w/Dr. Khouri was the next week, so I never ended up rescheduling and don't know if that was actually true or not.
It did prompt me to ask Dr. Khouri at my consult if he minded if I posted my experience as I went along. He obviously did not, lol! In retrospect, I think it would have been a red flag if he had.

I just have to say that I've grown weary of seeing a mean, spiteful remark followed with the defense of being protective of this site; as ironically it is that very negativity (even if the motives are pure) that drives many of us away. While it is fine to pose questions and concerns, I don't think it's too much to ask that posts be respectful of other members, along with the decisions we have made, and the unique yet similar paths we are on. If I were to make the rules (and I realize no one has asked me), I would say any post that makes a person feel targeted and/or degraded is simply not acceptable. Just my two cents.

I hope you're not assuming that I meant to be mean or spiteful, because I am neither. My motives are as pure as I stated earlier, "I remain protective of the website and the benefit it brings to us all". I've lost my sister to this beast, so I don't have the energy or desire to be mean.
This website has helped me through some of the most difficult times of my life. My reconstruction has been long and arduous, but my PS and I move forward, and there is a light at the end of the tunnel (My nipple reconstruction is scheduled for March!). We both know about the BRAVA, and have discussed it - along with every other possible reconstructive procedure known to medicine. My experiences have prompted a medical professional to refer me to talk with another BC patient who's having a difficult reconstruction.
My interpretation of the posts are that leeinfl is successfully done, psf is not quite done, and I was not clear on wisconsinrandi. I hope they, and all the women who choose this - or any other method - are ultimately successful and pleased with their results.
Yes, there is some disagreement on this thread, but not outright disrespect or so much negativity that one would be driven away. Let's stick around and wait for the success stories.

I am so tired of this...if anyone feels this thread is "advertising" for Dr.Khouri,don't visit it.I do not feel the NOLA thread or the Charleston thread is advertising for those doctors, why is this advertising for Dr.Khouri? I have never met the man,I have heard mostly good and a little bad about him.What I am interested in is the fat grafting procedure and if he is the one who is pioneering it here in the States,of course his name is going to be mentioned time and time again, as are Dr.Allen and Dr.Massey (and others) mentioned time and again for the flap surgeries.I feel it is being implied that Dr.Khouri's patients are doing this "advertising".If it is advertising to post about your reconstruction and mention the doctors name,then I guess many of the threads are guilty.......

Thanks Pandazankar - I just didn't want anyone on here to think for one moment that the fatgraftpatient website is in any way endorsed or sponsored by Dr. Khouri. It's simply a forum (just like this one...) that allows us to share our experiences and it's unfair to imply anything more than that. Those of us who have had wonderful experiences just want to share! It is so helpful to read what others have gone through, good or bad.
Just the other day I learned of a lady who does 3D tattooing. Her name was posted here on BC.org - if we were all so fearful of posting names, I wouldn't have found out about her either! I plan to visit with her soon. Her work looks fantastic! So come on....lighten up! Let those of us who have had happy experiences post about our good fortune in the hope that others might also reap benefits. It's taken me 4 surgeries to get to this point - if I can't share it with my fellow BC sisters here, then where?
So please - I don't think we need to protect this site. It's working just fine.

Let me make some clarifications, if I may ~
I'm not certain that anyone on this thread - except wisconsinrandi - has had a full, no implant, fat transfer only breast reconstruction, with or without the aid of a BRAVA device. I see some who had, just like I did, mastectomies, followed by tissue expanders, followed by implants followed by fat transfer revisions; or, some combination of these.
Also, I don't know of a rule on this site keeping anyone from mentioning their doctors' names if they choose to do so and/or have not signed waivers - and, for the record, I did not sign one.
The biggest problem I had here was when Dr. Khouri's staff was posting! If you read all the way back, this thread started 2/10/10, and by 2/16/10,his staff was posting here. THAT is what makes it different from the threads dedicated to NOLA and other practices. Then we had erika54 who was almost certainly planted here. The downside is that it made anyone else who posted for him sound like an advertisement, and I am not the only one who felt that way.
All of us on this thread have either had, or want, or are curious about fat grafting, either for revisions or full reconstruction, either by a brand name or any other name. A rose by any other name would smell just as sweet, no?

And when people bitched about them posting INFORMATION,Dr.Khouri's staff stopped posting.I for one was happy to have them answering questions.I don't know what the big deal is with this.If they were posting saying "call us for more info or for an appointment" that would be different.But they were correcting misinformation about the procedure.And if a person on this thread sounds like an advertisement,that is your interpretation,just as I am interpreting your posts as inflammatory and rude.You started this with your comment about Alexandra-aa's site,which is not even a part of BCO.If you don't feel this thread contains pertinent information, you could start another thread with what you feel is appropriate.Also psf is indeed undergoing complete recon from double mastectomy.Which if you were reading all the posts,you would know that.I won't respond to any more posts in this context,so fire away at me.

This isn't you're 'normal' fat grafting procedure. What Dr. Khouri has designed using the Brava system is a revolution in plastic surgery! No, I am not an employee of his, but wish I were... because I want this surgery!
I don't recall agreeing not to mention doctor's names... They do this at the hospital bc support group I attend. If the doctors are honest and good at what they do, what have they got to hide? It's not free speech and it's not supprtive of us bc patients who are looking for honest answers to our mnay questions. I get chastised for mentioning anything to the group which doen't promote the medical establishment. I'd like to start my own group, but where would new bc patients find it? They are reffered by their doctors...

" A rose by any other name would smell just as sweet" It is all exciting stuff. I find it extremely exciting that there are MD's being trained all around the world. The more the merrier. I'm very glad it isn't just Dr. Khouri doing this (the man is way too busy as it is). The more this option can be a reality for every woman, the better. Some will choose different options and that is fine.

Maria, I want to echo what the others have been saying... that the only reason this is a "Dr. Khouri-heavy" thread is that he's the one who came up with the whole-breast-reconstruction version of fat grafting -- so obviously he will be discussed quite heavily! As more surgeons are trained to do this technique, it won't be as Dr. Khouri-heavy.
For instance, my plastic surgeon (in metro Atlanta) is going one-on-one training with Dr. K in February. At this point, I have had a mastectomy with no reconstruction (no TE's, nothing). Mine will be a case of a complete rebuilding of my breast with this technique. I can't wait to be able to post as I'm going through it. If it works out as well as I hope, I'm sure you'll think I'm on my surgeon's staff posing as a BC patient, I'll be so excited to share the news! LOL
I, too, had no problem with Dr. K's team posting here as long as they were answering questions as honestly and accurately as they could (which, from what I could tell, they were). All of their posts were educational and informative -- not advertising.
I agree that erika54 woman was odd and if she was a member of Dr. K's staff posing as a BC patient, then shame on her. But it'd be crazy to slam the entire practice because of one loopy person.

Just an FYI - the American Society of Plastic Surgeons has a ton of information on their website about fat grafting. Here is one specifically about fat grafting to the breast:
One thing that stands out in several articles: "Studies indicate that results of fat transfer remain dependent on a surgeon's technique and expertise."
This one goes a little deeper into the role of stem cells that are present in the fat grafts and their potential to further heal/repair...

Haven't checked this site for a few days...wow...interesting discussion. I would count myself as a complete reconstruction by fat grafting method x2 (both breasts)...yes...Dr. K was my surgeon and I didn't sign any name disclosure waivers...
my right side is finished, my left has one more graft to go, I believe. Great results....right side has about 97% feeling back...left side is lagging a bit behind that. I guess I have to apologize for singing the praises of Dr. K and this procedure...but it is so hard NOT too...given my results! I am just so grateful to have found him.
Also...have to say Alexandra....your website was invaluable to me as I was going through the process....much more detail about what to expect and also, some of the pictures really helped me to have realistic expections of what to expect just post-surgery...helped me not to freak out! Thanks for starting it!

Has anybody had complete reconstruction using fat grafts with a Dr. other than Dr. R Khouri?
I see Dr.Coleman and Dr. Del Vecchio on his website. Just wonder if there is any information to support these Dr.s that are a bit closere.

Just wanted to post my experience with Dr Khouri. I just had micro fatgrafting at his surgery center in Key Biscayne. (Thanks for the headsup on Palmetto Starfish!).
The entire experience could not have gone better. Like it was said above...if someone takes this as advertisement for Dr K, so be it!! I feel after a solid year of doctors who are just doing their jobs...I am thrilled to sing the praises of a doctor who is so wonderfully passionate about his work. He loves to make women feel beautiful. He is enthusiastic and encouraging and positive. His office is brimming with patients who are so excited to share their experiences. At one point he yelled down the hall.."Julia, show her your resuts after we fixed radiation damage". This patient from another room who was leaving flies into the room, hiking her top up so excited to show how beautiful she was again. She spoke rapid Spanish with Dr Khouri, and I have no idea what they were saying..but they were both laughing and smiling. It was funny, and heartwarming.
I just finished a long year of treatment. I bought the Bravas, wore them a little while...but decided I wasnt in terrible shape with my implants after radiation, and wanted a break from treatment...of any kind! Dr Khouri listened to me. I kept my implants and he grafted all around, smoothing radiation damaged areas and releasing scar tissue. I had some serious assymetry from the radiation, and he fixed all of that. I am only a week out, so I do not know what will "take". What I do know, is that I had one of the best in the world working together with me. If only all of the doctors loved their work as much as this man...what a different experience this medical system would be!

geewhiz - Glad you had the same positive experience the rest of us have had. Dr. K is amazing and the Key Biscayne surgery center is soooo much nicer than any hospital I've been to. We have similiar procedures - with the exception that Dr. K replaced my implants with smaller ones and did some major work on my pockets. I've also had fat grafting all around making the breasts look natural and symmetrical. I don't feel like I've lost anything! I know that sounds unbelievable, but I've posted my photos on Alexandra's forum so feel free to see if you can see any difference. Aside from post-op swelling, I really think I've kept if not all, than certainly most of the fat he injected. I've had two rounds of fat grafting and both times, the fat held. No lumps or hard spots anywhere and the breasts are softening up nicely. Hope you have the same great results!

It has been very quiet here,hope everyone is doing welI.I met with a local plastic surgeon yesterday to discuss his placing tissue expanders when I have my prophylactic mastectomy.Before the surgery I will see a PS in Connecticut who does fat grafting .She does not use the Brava,she prefers tissue expanders.She is affiliated with Dr.Passaretti who I have heard is wonderful.It will be interesting to meet with her and also to hear what the local doctor has to say after his conversation with her.I am a little worried about the tissue expanders, have heard how painful they can be.But I was concerned about using the Brava too.....Any of you who have had expanders,I would love to hear how that went for you.There is a doctor in Boston who uses the Brava,so he is another possibility....it really feels good to know there are a couple of options.

pandazankar - I had TEs placed immediately after the MX and I believe they were the worst part of EVERYTHING I went through. It felt like a heavy, steel plate was in my chest. I could only take very shallow breaths (which had me on my way to pneumonia super quick!) and it was just so painful. I remember crying despite the morphine and immediately trying to stop myself since the crying made it hurt more. The fill-ups weren't horrible - just soreness for a couple of days after each one. I'm sure other ladies have had better experiences with the TEs but if I could do it all over again, I would NEVER have them put in. The residual damage and discomfort with the pec muscles is something I wish I could have avoided as well. Knowing what I know now about Brava and fat grafting, I would have definitely gone that route. Can't turn back time, but I don't think I will ever forget the TE experience......

I have been trying to read everything I can find about tissue expanders and it all says that the pec muscle will go back in place if expanders or implants are removed.But you have implants still correct? And Dr.Khouri put fat all around them? If I understood the PS correctly when we talked on the phone,she does one or two graftings and then removes the expanders and does a minimum of two more graftings after they are removed.I am a huge baby about pain....

It's not pain, but because the TEs and subsequently the implants were placed under the pec muscles it just feels uncomfortable doing things I used to enjoy. I know some ladies have reported real pain - that's not what I have. I'm lucky in that I don't have pain, but I live in Florida so I'm in the water all the time and swimming (breast stroke) is awkward and strange. First time I tried, I thought I was going under - surprise, surprise! I'm also a personal trainer and lift weights - that's where I have the most "discomfort" and where it has affected me the most. A lot of exercises and lifts I just don't do anymore because it's so uncomfortable. Since Dr. K replaced the implants with smaller ones, the muscle flexing or "jumping" isn't as bad as with the larger ones, but it's still there. Not pretty and again, feels weird.
Since your implants will be removed again, hopefully your research is correct that the muscle returns back to "normal" - that would be great!
Yes, I still have implants and Dr. Khouri did 2 fat graftings which he placed all around as well as add to the breast mound.
Your PS is planning on at least 3-4 graftings? Do you know which areas she plans to lipo? I can tell you from my experience that the thighs (inner & outer) were a piece of cake compared to the lower back (muffin top) area. So you need to be prepared for pain in the lipo areas, at least for the first couple of days. Again, some areas hurt more than others. Numbness and tenderness lasts for several weeks, sometimes months. Don't mean to scare you because I would still take the lipo sessions any day for the beautiful results I've got in my breasts - just want you to be prepared.

I have a lot of sensitivity on my old mastectomy side. One PS I saw told me opening the old scar might help,but might not.So the TEs concern me,but they will only be in a short time and no implants,so if this is what I decide to do,I hope there will be no long term effects.Was there a reason you kept your implants and did not have Dr.Khouri do complete recon?I won't know for sure how many graftings until I see her and she said that may change during the course of things.I am not looking forward to the lipo.She did say she prefers abdominal fat.That is another thing I want to ask her about when I see her,I had not heard that before.Even with the stretching,whether with the Brava or TEs and the lipo,I still find the fat grafting much more acceptable to me than the other choices.When will they invent a magic wand that gives us beautiful healthy breasts with no pain or inconvenience??

pandazankar- I haven't had fat grafting but have been reading this thread with interest as I do have pain with my implants. I don't know if it is coming from the implants themselves or the pec muscles constantly stretching. I would definitely ask your PS whether your pecs will be relocated and if that will be permanent. Like leeinfl, the sensations of the muscle when it's flexed is very bothersome. Did your PS tell you why you have to have TE's? I'm wondering since I've seen others who have had the MX with a one step procedure where the implants are put in at the same time. I was totally unprepared for how the TE's would feel. Kind of felt like I was wearing a turtle shell for 3 months.

I will not be getting implants.The tissue expanders will be used to stretch and make new skin,the same as if I was having implants.The difference is fat from my abdomen and thighs will be liposuctioned and put in the breast area instead of implants.The tissue expanders will be removed after one or two graftings.My new breasts will be totally my own fat.Some doctors use the external Brava to stretch,the doctor I will see in Connecticut prefers using the tissue expanders.I will definitely ask what effect the TEs will have on my muscle.The local PS I saw yesterday seemed to feel that since I would not be getting implants,there would be no problem with the muscle.He mainly does implants,one reason I chose him to place the tissue expanders,he has a lot of experience.I am grateful to you all,this is really helping me add to my list of questions!

pandazankar - Initially Dr. K was just going to "fix" a dent in my right breast, but then suggested a "redo" in order to get a good result. I never thought to ask him why he used the smaller implants - maybe because I had such a large pocket already (TEs were filled to 600ccs) - he already had to do quite a bit of work on them. Maybe he didn't want to totally shock me? LOL...
I think I remember reading that fat injected under the muscle isn't as effective (viable). I will ask him when I see him in early March.
Growing breasts from your own fat isn't exactly a magic wand, but sure is better than all the other options out there!

Lee, does the transplanted fat soften a bit? I am a month out...bruises finally gone!! But things are a little firm. I love the way things are looking. It's so much better!!

Oh,I see! You had a large pocket,so it made more sense to use an implant and fat.Does it feel better with the smaller implant and fat than it did with just the implant?I agree,this really is a wonderful way to regain our breasts.The doctor will be putting the fat between the muscle and the skin,the expander will be under the muscle.She removes some saline and does the first grafting.At the next grafting she may remove more saline or the whole expander.I can't wait for my consult,so I have a better idea of what to expect.geewhiz,I know psf said her breasts were softening up nicely,look back through her posts.
