Question: DCIS Upgraded to 3 with Necrosis

An addendum to my path report upgraded my DCIS  (solid type) from intermediate to high grade with necrosis.  Does anyone know whether this is aggressive like the comedonecrosis type? I can't seem to find anything definitive.  Any help would be appreciated.

    My onc said that high grade with necrosis meant that it was not a question of would my DCIS become invasive but when would it become invasive.

    Does that help?

    My grade 3 was comprised of primarily of comedo cells with necrosis, so I would not be surprised if that is the type of cell that makes up your grade 3.  The cribiform is not usually as aggressive as the comedo.  I had both in my pathology.

    I don't know the answer to your question, but I am curious too.  My understanding was that necrosis is a bad sign because it means the cancer is growing fast enough that it cuts off its own blood supply (and then the part that has no blood supply dies).  But maybe that is only comedonecrosis?  I really don't know.  I think about asking my oncologist from time to time but never remember when I am there.  The pathology from my stereotactic biopsy was solid and cribiform with central necrosis, intermediate grade and the pathology from my lumpectomy was solid and cribiform, intermediate to high grade (I guess they got all of the necrosis in the biopsy).  My DCIS was small, but I was very strongly encouraged by two medical oncologists and my surgeon to take tamoxifen, which I am doing with minimal side effects.  I think they were particularly concerned because of my relatively young (for cancer diagnosis) age -- I was 40 when the DCIS was found in my first mammogram.  As far as I remember, no one ever said much of anything about the necrosis.  So far, it has been about a year and a half since surgery and over a year on tamoxifen and all is well.  Good luck!!!

    i too have the exact same diagnosis. the first bioposy was high grade but the final one came back 2 and 3 with comedo type as well. even though it was very small and wide margins, they too suggested tamoxfin since i am high ER and PR at 90 and 100 percent. I have have little issue with takng it except I hate swallowing that pilll. it constantly sticks in my throat. ARGh AND it tastes bad.. but I would rather be safe then sorry even at 60...

    except the annoyance of where the nodes were removed (why they took 2 I dont know) and some sharp random pains in the boob. i feel great.

    i too had the comedo necrosis, grade 3. And ALH. I opted out of Rads... went with BMX. Rads Onc discharged me....feels I only need to be watched by my BS every six months for chest wall recurrance.  I was told I had a 1-2 % chance of recur, no need for PETs, MRI's....

    Any thots? I had a 4mm tumor that showed up on Mammo as microcalcs.... I am still trying to figure if I need to see Med Onc as everyone is advising me no need to as it was not an Invasive Cancer. But if it recurs there is a 50% chance that it would be Invasive....

     Arrggg.
