RM- private hospitals who refuse to admit risk losing their medicare eligibility and this is frowned upon by JCAHO, the accreditation body for hospitals, particularly in areas where there are no public hospitals.

California spends about $10 billion PER YEAR on covering illegals from Mexio, C.A., Asia and Europe. Our services that are being used by them are mostly the medical, education and penal system. I did find that the US gov't does as reimbursed us $1 billion towards medical ... but it was over a 4 year period ... so we must provide services, we Californians pay an estimated $1000 per tax payer for them, the State only gets reimbursed $250 million per year, thats 2.5% of the cost to us

RM- I don't doubt that there is money changing hands when dealing with ambulance companies and indigent patients. I used to live in a town where people who went to the ER for non emergencies either walked there or took a bus. They clearly didn't have what most people would consider life threatening emergencies, but there were no public clinics and it was the best they could do for themselves and their kids. It's really sad when kids are involved.

RM and Amy: Thanks for the info. The reason I asked in particular about what happens to patients who don't pay their bills is because it now seems obvious that public hospitals (especially) need to make up the shortfall, and thus must charge higher than necessary fees for health services. Without doubt private hospitals do as well, if they are forced to take uninsured patients sometimes. Therefore, those who "can" end up paying more. And it's a continuum -- the higher the fees, the more the insured also pay, leading to higher insurance rates.
Not so different from our system, in that those who pay taxes to support healthcare subsidize those that don't have enough income to pay taxes. But I guess the real difference is that we don't, on the whole, resent this! Healthcare for all is engrained in our psyches.
ljl: the article you posted from the NYT was written nearly 3 years ago. Wait times have improved significantly, at least in Ontario, since then. However, the debate re public healthcare versus private clinics does continue. The biggest fear has been that the best doctors will gravitate to private clinics, but of the docs I know (and they are some of the best in their field), none has ever expressed any interest in leaving the big teaching hospitals in order to make bigger bucks privately. They are more concerned with the prestige (and knowledge gained) by working in the teaching/research environment.

My Kaiser doctor loves working for an HMO ... he says he spends his time operating and with patients, not on the phone with insurance companies and trying to worry about malpractice suits, paying office staff, billing, etc. ... One of my dad's friend's wife needed brain surgery. They consulted with a Stanford doctor (Stanford U being expensive and prestigious), he advised them to go to Kaiser!! He said most brain surgeons are doing a few a month where Kaiser docs are doing several a week, so Kaiser surgeons have more hands on .... I know my breast surgeon ONLY does breast surgery.
I can see that we probably will move to more public funding or public insurance but my hope is that the public insurance is not just for poor ... it should be for everyone. My paycheck only goes so far and the price of insurance is high ... Getting federal money for things like this should be for everyone, not just poor and indigent. It would save employers money even tho we'd pay taxes for it .. the problem with so many gov't programs is that the middle class and lower middle class don't qualify for the programs.
When you go to fill out money for food stamps you are asked the value of your house ... well, hello? We owe more than it's worth. You are asked about your 401k that has sunk ... so if you have anything, you don't get it ...you have to lose everything before they help. It would be cheaper to help people stay on their feet than pull them up off the floor.

I'm sure every place on this planet has its issues. There are places better than the US, and places worse than the US. Depending on who's point of view it is, and what day of the week it is. But..for right now..we're here. The US needs major improving on the health care issues and revamp the insurance companies. They have far to much say in our personal health and receive far to much money for what they give us. I do think there are better plans out there than what is offered in this country. That's my opinion. For those of us that have a great insurance plan, and company benefits, and not much health issues, well..they might feel differently. It's all a matter of who's eye's you're looking thru.

Are you sure about your first statement about the elderly coming legally and getting benefits? I don't believe that is correct. You have to have paid into the system to draw out of the system. And you can't just do it for a few quarters. A person must qualify to receive SS by putting $$ into SS first--out of their job taxes--and the employer pays in as well. At least in this neck of the woods and all the work I've done on figuring out SS and Medicare, everything I've read in government pamphlets and documents--is pay to get pay out!
Mexicans in CA who work for an employer must have a SS card (unless the employer is unscrupulous and pays cash under the table)--the SS card may use someone else's # or be a fake one--the SS taxes are taken out and applied to that #--you are right the "illegals" (most are Mexican in my area) or Mexicans never draw that SS--it remains in the government's coffers. So, technically, the "illegals" do us a favor and gift us with their SS earnings that they never draw on because they are not US citizens. We could go further and talk about the emigrants from other countries who receive a pension from that country (and never report it--as their relatives get it at their address in the foreign country, cash it and send a money order to the not-a-US-citizen in this country--these emigrants also worked here with legal green card and thus drew SS also--which would have been reduced had the SSA known about the other income when they were employed or after retirement! These were white people. So there are many ways our system is broken and the blame should not be put on the shoulders of the non-citizens who come here to help their family survive. Some people have no consciences' no matter what skin color they have or what country they come from. My experience with Mexican workers is they are the most hardworking group of people I have ever seen and they are dedicated to their families. I am proud to call all those I know by name my friends--sometimes I wish my family members had morals and ethics like those from Mexico, CA, & SA.
I don't believe the lack of immigration problems in Norway is what funds all the services under their socialist system--it is their taxes. Unfortunately in the USA people want all the goodies without paying for them--if we want the government to support these goodies, we have to raise our tax base to 50%. That is what my Finnish relatives pay out of their salaries; and Norwegians do the same. Too many of us are selfish and don't want to share our goodies. We live in that wonderful state of meritocracy--not really a true democracy! So if you want what the poor get then you will need to fork over more of your paycheck. I doubt most people would be willing to do that.
You are so right about having to lose everything in order to get help. Even the elderly living at poverty level have to use up what they have. Although Medicare or Medicaid won't take away the house they own, they will be the first with their hands up to get what's owed them when the elderly person dies. Don't worry, if there are assets, Medicare and the government will get paid first!

I think I firstly said Asian. Mostly I see it with Philippine immigrants .. legal ...so, I looked it up and now see that the Philippines were a US Territory, they or their husband must have worked the minimum quarters while it was still a territory. That's in regard to SS ...
But immigrants, legal ones, come here and qualify for benefits such as food stamps, Medi-Cal, welfare without having "paid" into the system. I am at polar ends with some of you when it comes to immigration ... I see it as people continuing to come here, leave their countries that are riddled with corruption and come to ours ... we middle class, and the weathy, too, are stuck with high taxes. I believe that our country should have serious talks with countries like Mexico and the ones in S.A. and C.A. that they should really take better care of their citizens ... spread the wealth, re-distribute it rather than come here and demand services that we can't even get.
There's always someone worse off than we are ... but we'd be better off if we had sensible immigration laws and didn't make the great State of California pay the bulk that we do. If the citizens of the USA want illegals to have benefits, then reimburse 100%..... there was a vote in the 90's .. Prop 187 ... Californians voted to not pay for the education or other public services of illegal immigrants .... the feds turned it over.... so I am not the only one on my this side of the planet. Like the article said 60% of the uninsured coming into LA's hospitals are "alien" residents and illegal immigrants. Now, it would be fine for us to accept that if we also allowed to send the bill to their country, that would solve the problem in my mind!!

RM- the example Hawaii example is a historical tourist site and they use the term "leper patient" not leper to describe people. There was a time when disabled people were called cripples. We don't use that term any more because it's unkind. There is a difference between using perjorative words as adjectives and nouns. Republicans use the term "illegals" much more than democrats. We seem to have more respect for people. The USA headline is horrible and I hope a lot of people complained about it. Why do many republicans act like some people are less than human? They do it with illegal immigrants, they do it with gays, they do it with poor people. Many talk about the poor, who have trouble feeding their families not paying taxes and how that's unfair to them. I guess I just have a hard time when disenfranchised people aren't treated with respect.

We disagree and that's fine, but it's not for lack of understanding on my part. I have a lot of compassion for people who are less fortunate and who were born into countries or situations where they can't get adequate care
Where did this come from ? I've never questioned your intelligence or your education. I was just stating my opinion that it is not possible to take care of everyone due to the scare resources as much as we might want to. And that it's not fair to people who wait in line for years to come here to allow other people to cut in front of it.

I know answer to this, my uncles and aunts who came here when they were 60, receive SSI benefits. And they also get Medical/Medicare which is much better than my parents Medicare and they worked and retired here. Don't get me started on this

I guess I don't see the word leper as perjorative .. there are lots of people who use the word leper to describe people with that disease. I see the word leper just as I do in the bible, patients who have a horrible disease that's not their fault, where they suffer even after being cured because of disfigurement. If you look up the word leper on the internet, you will see it is a widely used term as the disease is still in existence in India. I could never see the word leper, a medical term, to be a word as disgusting as the N word.
I don't think thats really fair of you to insinuate that because I call them illegal immigrants, which is what the United States Government calls them, that I am not treating them with respect. You may disagree with me and I with you but that is not fair to say that I see them as "less than human" ... I think those are pejorative statements aimed right at me. You know Amy, my daughter had a voluntary art project to do if she wanted. The project was called WOW... now you can take a picture of a sunset, draw, make a movie of toe nails growing. But my daughter and I decided to focus on the homeless shelter and food kitchen in our area. My daughter and I raised enough money by going store to store asking for donations for school supplies for our local shelter...so the kids would have backpacks and the supplies to go in them. One angel from this site, LuAnn, even sent 3 boxes of goodies to help. There were 30 kids. We don't know if they were legal or illegal or purple. We did such a good job we had left over money and went and bought hygiene supplies, gel, hairspray, tampons for the families that get to live there for 9 months. We intended to make a cd of pictures showing the differences of what dd has and those kids don't, like "Wow, people need some help or Wow, look what a little help and alot of love can do" ... but after eating at Soup Kitchen with them, playing with them, hanging out, bringing stuffed animals, clothes from my dad to give to them men for job interviews, clothes from my closet ... we took tons of pics ... and they look happy, cared for, everyone looks like us ... and we never made the cd ... yes, they live in a shelter but that doesn't translate well onto our camera, the kids beds looked like hers filled with stuffed animals .. this was not Skid Row ... So, no, we didn't finish the project, she was happy with what she and I did together, to us, the project was complete. It was ok to not turn it in ... It was a voluntary project. We are scheduled with the Shelter for an appointment in June to do it again but this time to get BEFORE they start school.
But we donate our time, we visit the elderly. We don't get points at church because no one there knows we do it or grades or brownie badges (because she isn't a Brownie, mommy works 6 days a week and can't get her to the meetings).
I really may have opposite feelings about things but I am respectful of other human beings and I reach out to help with actions and word since I have no money. When we had money, we took 5 kids names from the giving tree and made sure to fill their wishes plus a Xmas outfit, including socks and underwear, making sure it would also be fit to where to school so the outfit was not a one time outfit.
Sometimes, it seems to me, that you think I am some rich b*tch who has had everything handed to me. I have worked hard, not even had the opportunity to go to college, and made a life for myself despite having endured an abusive marriage. Not all Republicans are rich. I just want to keep what I work for.

You are right. But my fear this that as the result of the reform it will be the same except instead the insurance companies in the paragraph above it will be the government.

I honestly don't see why people are so afraid of having some government committee decide on their health care (and I did read the committee report on the assessment of the value of something or other I took) and haven't a similar revulsion to some insurance company flunky making a decision.
Maybe I've written about this before, certainly I've thought it. In the early 90's I was a lower level manager in a community hospital - about 400+ beds, mixed use. We had 5-6 upper managers (CEO, finance, personal, etc.), about 15 mid level ones like me - most were nursing unit managers, 3-4 secretaries and maybe 10-12 in records and clerical. One of the secretaries did billing part time.
Whiile working there I watched a program about a hospital in Oregon, pretty much the same size and patient mix. They had a whole separate building full of people doing billing. And that is exactly where your health dollars are going and why the US spends more for less than just about every other industrialized country. Check out all the measures of population health - infant mortality, life expectancy, etc. It takes a lot of time and people power to keep track of what is used for each person and make sure the right company is billed and that they will approve it. From what I can tell government bureaucracy has nothing on what insurance companies can inflict. -

Just like some republicans wanted Obama to produce his birth certificate on demand, want to know who talked to Blagovech (which is fine) and are complaining they didn't have that information last week, we can't let naysayers come here and say jump, and we say how high. I guess I'm confused about whether your daughter's art project was supposed to be a parent/child project or hers alone. Whichever, it was a great experience and I think it's great you've given your daughter the gift of service. I've volunteered in shelters before and stuffed animals on beds don't make up for not having homes. Some shelters are better than others and just because yours wasn't the most awful, If you notice I don't say ALL republicans, I say MANY. I don't say who it applies to.To me, providing health care to only those who have insurance or can pay is treating people less than human. I am a cat lover, I have 4, and they get better health care, even if I have to go without, than some people.

Amy, almost any word can be used pejoratively, that doesn't mean that the original definition doesn't apply. To some ... the word Indian is pejorative and they prefer to be called Native American, yet I work near a reservation and that tribe calls themselves a "band of Indians" ... it's the feeling behind a word. Just because you may say someone is treated like a leper doesn't mean you have disrespect for lepers.
In order that you are not confused, let me explain: my daughter's project was to use the camera not the actions in front of the camera. She either had to draw a picture, write a story, take pictures, write a song or do a dance. It's a nationwide art project. When we contacted the shelter about her taking pictures they said, sure but how could we help them since they would help her. The "help" part of it sprang from that and it took over, so to speak. We know it's not the same thing as having a home but she is not old enough to write the story on her own and if I had helped write it, THAT would have been against the rules. The pictures my daughter took...well, you can't tell the kids are homeless ... they all had smiles, they weren't sad appearing, the ladies serving food at the kitchen looked just like the pic of the ladies serving food at her school. She took pics of kids eating at the soup kitchen and kids eating lunch at school ... you couldn't tell which was which ... Her pictures didn't capture anyone looking like they might be homeless.
Amy do you *ever* think you can see beyond your frame of reference ... you may not agree with me all the time but that doesn't mean I am always wrong. Maybe you enjoy the banter or the arguing or being critical of me. Have you ever read the book "How to Win Friends and Influence People" ? That book talks about how being critical of others just serves to make people dig their heels in. It also talks about how you may *win* an argument but you won't change opinions even if you win. I would hope that you could see that I try my hardest to help daughter be a thoughtful caring person ... that just because I may vote Republican doesn't mean I am a snobby uncaring person who needs etiquette lessons.

So the automakers bailout is still not a certain thing. Bush administration is trying to raid TARP to pay for it.
I am watching CNBC with everyone arguing but the person absent in this discussion is Obama. I wonder what his position is. I am surprised he is not there advocating passing Big 3 bailout ASAP. Any ideas why this is not the case?
BTW apparently I am not a minority among Republicans, latest polls show that Obama approval rates are rising among Republicans following his nominations but decreasing among liberals.

Let's stay on pointe here, shall we? Obama is the PRESIDENT ELECT, not the president, which is probably the main reason you haven't heard more about what he wants to/thinks should happen with the Big 3, ijl. Imagine the fine line he is walking by preparing for/letting everyone know what is happening transition-wise, but not actually trying to take anything away from the current (although lame duck) president. All Obama can do is make suggestions, not policy. That is the senate and Pres Bush's sticky wicket to sort out as his suggestions don't have to be taken into account.
At the risk of sounding confrontational, I'm just going to ask this: is your goal to be argumentative, ijl? That's what your comments come across as. Not against anyone posting here with a different opinion, but the manner in which you express it seems to call us (Obama supporters) all to the carpet, which is so not necessary. It's getting real old, real fast IMHO...

RM- I'm right about the word leper, but go ahead and use the term, I doubt you'll ever encounter someone who has leprosy. I think you're arguing for the sake of arguing, trying to find any instance where the word may be used. Some blacks use the N word when talking about each other, that doesn't make it right that you or anyone else does. People use prejorative words for gays, some gays do too, but I never do, because I want people to use the term I'm comfortable with, lesbian not queer, dyke, etc. I doubt, if I was black, I'd ever use the N word because I think people aren't necessarily bright enough to realize that certain terms said amongst a homogeneous group aren't transferable when a majority group is talking about a minority. I used to not understand rap lyrics and how they were modern day poetry and self expression, because of the language, but over the years I've come to appreciate why some of the words are used in some rap songs.(not ones about raping or degrading women...). I know you're teaching your daughter to be compassionate and I think that's great. You are giving her a great gift. Thanks for explaining more about the project. I was a bit confused. There's a wonderful book THE DOUBLE BIND by Chris Bohjalian abdout pictures taken in a homeless shelter. It's One of the top 5 books I've ever read. http://www.chrisbohjalian.com/bohjalian-double-bind.htm . I highly recommend this book and all his work.
ijl- You're mistaken about liberals and Obama's approval rating. The most liberal people, the far far far left (I only have 2 fars for my left) wish that he were going all liberal, the way Bush went all conservative and said f#$k the liberals and people who don't think like me. The rest of us realize the best way to build a coalition and get people to work together in these difficult times. I think his approval ratings for repubs are increasing because they believed the negative ads about Obama and where he wanted to take the country. They didn't believe when he said he wanted to reach across the aisle in a bipartisan way.

Madalyn, I totally agree with you when it comes to comparing govt. agencies with private insurance companies. Health insurance companies are grossly inefficient and wasteful. 25% of our premiums go to administrative costs, as compared to 5% in most single-payer systems. And what does all that money get us? Screw-ups and waste. I know most of us have horror stories about them. It would be one thing if the extra money we spent went to improved efficiency, but the opposite is true. Cleaning up the administrative inefficiency of private insurance companies is one of the ways Obama hopes to make insurance coverage more affordable.
I have to agree with Bush on the auto bailout. The consequences of letting the industry fail will be devastating I think. The ripple effect would be enormous. Even now, we're feeling it in LA, because the auto companies aren't buying ads on TV like they used to, so the networks aren't getting as much ad revenue. NBC laid off a bunch of people recently. And everyone has heard about the Leno deal. I know people don't think the entertainment industry is that important, but it does employ a lot of folks in LA. So, if it's already affecting us, it's going to affect other industries too. If nothing else, no one will have money to buy those import cars Senator Shelby is protecting.

Interesting--spread the wealth--one aspect of socialism. Many in the US are against any kind of socialism--even for our own citizens we don't want to share the wealth, re-distribute it! Guess it's that whole idea of "not in my backyard, or not in my neighborhood." Perhaps we should take better care of our own citizens before suggesting that Mexico, CA & SA should do that! Why should they do that? And why shouldn't we do the same?
Wonder if we could refer to those you address as "illegal" and "alien" as non-residents or not US citizens? It seems much more humane. After all they are not aliens--they are human beings, and many of them reside on our continent.
I use the term Mexican because many living in CA assume that the issues of benefits and free healthcare/education costs are consumed mostly by "mexican." It seems in CA that many people hear someone talking Spanish, and assume they are non-resident, poor, and sucking up free stuff that we can't get! If you have another term you would like me to use, Please what would it be?

I still can't believe the Republican senators--see also Hoover--played Russian Roulette with the working class auto workers last night. Of course they all have subsidized foreign car companies in their states. Even Toyota does not want this to fail. I am ill.
They continue to make me ill with their pompous pontificating. No thoughts to the jobs that might be lost...Amazing. This is not the time for union busting. I am not articulate this PM and my fingers are very painful but I had to post.
I am totally glad Senator--The fundamentals of the economy are sound--is not our President-Elect.
About the "N' word; I had a horrible time getting my son to see why he couldn't use it casually with others his age...dam*ed rappers. He stopped and has moved on, but it took me a while. A male teacher finally got his attention...boys do need dads...and he began to see the logic of what I was trying to teach.
Now that Obama is President-Elect, I pray for less "N" word use on the streets of Harlem, less absentee dads, less deadbeat dads and more family dad time.
