I've been experiencing itching and tenderness to my right breast. I've also noticed an descent size lump near the nipple. I'm very concerned. I need advice on what could be going on. I've experienced discharge from the nipple clear liquid.

Sounds like it might be an intraductal papilloma, which fortunately is completely harmless. But whatever it is, you need to see a doctor to get it diagnosed.
