Seroma draining from incision site - looking for wisdom
I had my lumpectomy 3 weeks ago, and literally had no instructions from my surgeon for post op care beyond, remove the bandage after 3 days. Diagnosed as IDC, Grade 3. Tumour size was just over 1 cm with 1.5 cm margins, so I would guess that the mass removed would be about 4 cm. also SNB removing 2 lymph nodes. I'm not a great healer, and had a packed wound for months with my hysterectomy years ago. In this case the SNB incision has healed great, with some swelling underneath my arm but not too bad. The tumour was at 6 o'clock, very low on my breast, at the chest wall - in fact I almost didn't bother to have it checked because I didn't think it was actually in my breast at all!! The incision is just above the mammary fold under my breast, close with dissolving sutures. About a 1 inch section has not closed and after a few days was draining as a slow steady drip, which stopped and started. I had a phone consult with the surgeon's student, and a photo to the surgeon who said that everything looked ok, no infection. Last week it stopped, and I thought it had sealed up, but it burst open one night and I literally peed out probably close to a cup of seroma fluid. Not through the whole 1 inch, but like a squirt gun. I massaged and manually expressed until nothing more came out. After more weeping that night, it stopped again, until last night when it happened again, when I was cleaning the incision area (I think I poked it too hard!). This time the fluid was more tea coloured, but still clear and no smell or sign of infection. Breast is otherwise fairly soft, no hard knot of a seroma under pressure. I think all the weeping has been like a safety valve. Again I expressed it until empty, but I am thinking now I was too aggressive, as my breast is pretty achey. My nipple is also hyper sensitive, and the veins in it seem to swell up and then subside. My daughter thinks this is because of the Sentinal node removal and lymph having no where to go now.
I am plus size but have fairly small B cup breasts, so nothing really weight wise pulling on the incision or lump site. I'm thinking I should baby it now and let it heal seroma or no but I'm not sure if I should try and compress it or just leave it. I can't wear any kind of a bra. I tried to use a sports bra but the band rides up to my incision site and irritates it even with padding (I tried maxi pads even), which is why I think the one spot didn't knit together. Any idea what I might use, it would need to be something in the 3X size range.
Thankfully path came back with clear margins and lymph nodes. I'm waiting for an oncology consult to determine what if any next steps there will be but I'm seeing radiation is pretty common, how does the healing typically affect any timing for radiation. I'm also 62, so don't know what hormone therapy if any they will use I am Est+Prog+ HER2-.
Any experience or wisdom would be appreciated.
