Shoulder pain and arthritis after mastectomy/DIEP reconstruction

Four years after a successful DIEP surgery I am having shoulder pain on the reconstruction side: arthritis, tendonitis, etc. I read that arm/shoulder issues are a common outcome, as the mastectomy can mess with the movement of the clavicle. But I'm wondering if part of the problem is that the DIEP breast is heavier than the other (C-D cup, probably a pound heavier) even though it's the same size visually. Anyone had this issue and found a way to deal with it? I may have to go for breast reduction if I can't find a way to manage the symptoms.

    Did you ever have any lymph nodes removed? I had bilateral mastectomy with no reconstruction - and have some issues with my rib as well as periodic clavical/shoulder pain or issues, I can see in the mirror that my left (affected) side is askew, though doctors are very dismissive. Technically both of my sides are "even" and less weight from no breasts shouldn't cause issues - yet it does. Mine are due to lymphatic issues. My mastectomy was also in 2017. Obviously your case seems more likely for uneveness, just saying - I should be "even" but also have problems. I find it to be very frustrating.

    No, I didn't have lymph nodes removed.

    Have you tried PT?
