3 years since 1st DX, now BIRADS4 on other breast

It's been almost 3 years since my original DX. I went in for my annual mammo (on the R breast) and US today. Nothing suspicious was found on the mammo (3D), but the US noted something on my right breast. So I'm scheduled for an US guided biopsy a week from today. Whatever it is is right behind the nipple. (The tumor on my left breast was also right behind the nipple. Biopsies behind the nipple hurt like hell, but I got through it once so I can do it again)

I have my annual appt with my BS on Tuesday. I think I'll inquire about removing the right side.

Here is what the report says:

Ultrasound of the right breast was performed. In the subareolar region, a
fairly hypoechoic shadowing lesion was suggested although very difficult to
image through the areola. There appears to be a possible 8 mm solid
shadowing lesion and slight nipple retraction. There is no corresponding
lesion identified with certainty on mammography. The remainder of the right
breast and axilla appear normal. The left chest wall was scanned where the
patient feels some thickening around the scar. A minor degree of
hypoechoic scarring was noted. There is no left axillary lymphadenopathy.

ACR BI-RADS® Assessments: BI-RADS 4 - Suspicious (Overall)
Right breast Scr Right Tomo: Right breast is BI-RADS 1 - negative.
Right breast US Right Compl: BI-RADS 4 - suspicious abnormality in the
right breast. Suggestion of a 8 mm right retroareolar mass with shadowing
and slight nipple retraction, difficult to image but persisting during
repeat scanning.
