Flat spot

I had breast implants immediately following my BMI in 2014. Had to have them redone two years later because one had fallen and both had started to spread out. Had fat grafting in 2016 to help fill them out some. I am still not happy with them as they have started to migrate further from the middle of my chest, but with the right bra, they look ok.

My left breast seems to be flattening out a bit. Does this happen with fat grafting sometimes years later, is it the padded bra, or is there a bigger problem?
