DCIS in Right Breast 2011; Routine 2019 MRI BIRADS 4B

Hello - I am writing on behalf of my sister who was diagnosed in 2011 with DCIS in the right lower breast and had a successful lumpectomy and radiation. In the years since, she has been cleared but has had 1 BIRADS 4 diagnoses on the right breast and 2 BIRADS 4 diagnosis on the left breast all turned out negative. The last in 2015 was a " 6 mm lesion nodular demonstrates suspicious enhancement kinetics" in the upper right breast" which after an MRI Biopsy tuned out to be scar tissue.

In all, she has already had 2 lumpectomies and 2 biopsies of the right breast and 2 biopsies on the left. She is no longer living in California where she was last treated and is currently working in Jordan. In January 2019, she had a routine Mammogram and ultrasound that came back normal but the January 20th MRI report showed "Small area of linear nodular non-mass enhancement seen in the immediate retroareolar tissues of the left breast measuring about 6.5 x 7 x 8 mm. " The assessment is as follows :"Small area of non-Mass enhancement seen in the immediate retroareolar tissue of the left breast. Assessment category BI-RADS 4B. Biopsy is recommended. "

Her Jordanian oncologist asked for her last 2016 MRI images to see whether this is a new finding . Unfortunately, my sister has everything with her but that one disc. Finally after several weeks, we have gotten the 2016 MRI images and report to the Jordanian oncologist and she has now assessed this as a new finding. This seems strange as the 2016 MRI report does seem to refer to comparison exams from MRI's dated 2015 and 2013 and has the following statement of something that sounds similar- and is exactly the same size in the left breast: "There is redemonstration of 0.6 cm nodular enhancement in the retroareolar breast, unchanged from the December 9, 2013 study." This 2016 MRI came back as benign, BIRADS II in left and right breasts.

She sees the Jordanian oncologist this Thursday and is confused as what to do. The oncologist feels that - in the worst case scenario - it is non-invasive - DCIS - but probably will want to biopsy even though she seemed unsure the last time of what to actually biopsy. A second ultrasound spot the MRI revealed nothing.

Again, she's had so many biopsies and het last California breast surgeon felt that some had been unnecessary. This breast surgeon says she cannot offer an opinion even though we sent her the report, as my sister is no longer a patient.

Any suggestions from the community about how she might be able to get a second opinion back in the States would be much appreciated. Thank you and good luck to all!
