Nipple duct sore on nipple. Paget's?

I'm a breastfeeding mum. My little one is 21 months now. In early November I had a blocked duct. I thought it was due to the irregular feedig pattern and coincided with work etc. It blocked a couple of times. When it finally cleared I was left with a sore spot on my nipple. I recall.this happening in the past but it went away. This time the sore spot surrounds a single milk duct and is raised and inflammed. Milk still flows but doesn't really jet. There's no other discharge. I had a 10 day course of antibiotics and some topical treatment with miconazole and also hydrocortisone. I felt there was some improvement but I'm. Not convinced either. It's painful to nurse from the sore spot. I cannot find ANY information on this type of nipple lesion. I'm stressing about the feel of my breast which goes up and down throughout the day. I have a breast clinic referral. I'm an anxiety sufferer so struggling greatly.
