I'm just wondering if anyone had a similar dx to my moms? I'm finding online that comedo necrosis is generally a sign of high grade IN SITU so I was wondering if this was unusual or just something I'm not understanding correctly?

Any information on this topic or even this DX would be super appreciated!

Thanks so much for the help and guidance all you beautiful ladies!

Tevyn, it is very common to have areas of kinds of in situ cancer and/or other atypia ALONG WITH the area of invasive cancer. She has both DCIS and IDC.

oh alright, so the comedo necrosis may have something to do with the invasive part of the cancer?

Sorry I'm having some trouble understanding. Is it generally a bad thing if IN SITU is found along with IDC?

I think many if not most DCIS has some comedo necrosis. Mine did. And it's very common to see DCIS and IDC in the same breast.

tevyn - comedo necrosis is an aspect of the DCIS portion of your mom's breast cancer, indicating higher grade and proliferation, the presence of cell death (the necrosis part), but the DCIS is still confined to the duct. Regardless of the grade and comedo necrosis, the DCIS portion of your mom's breast cancer is still stage 0. It is common to see both IDC and DCIS together at diagnosis, if you read signature lines of IDC patients on this site you will often see that there was DCIS present as well. The IDC portion of your mom's cancer trumps the DCIS and is what will drive any treatment decisions beyond surgery.

Many of us have DCIS with comedo necrosis.  And DCIS. Your mom's DCIS is grade 1. That's good, better than grade 2 or 3.

She will find out a lot more with post-surgical pathology. Important things are whether it is HER2+ or -.  and whether it is positive for responsiveness to estrogen and progesterone (ER/PR).  If negative HER2 and positive ER she will have an Oncotype Dx test, most likely, that will guide treatment.
