Tamoxifen & Mirena - uterine lining / bleeding . . .

When I started on Tamoxifen almost 4 years ago (September 2014), I had a Paragard (copper, non-hormonal IUD). After 6 months on Tamoxifen, I had prolonged bleeding/clotting and uterine lining thickened. I ended up having a uterine lining biopsy (benign) and replacing the Paragard IUD with a Mirena IUD. My tumor was ER+/PR+, although only weakly PR+. My oncologist OK'd the Mirena on the grounds that the Mirena only releases a small amount of PR locally, and the PR should help reverse the uterine lining thickening.

It took a while but 6-12 months later, bleeding stopped and returned to occasional spotting levels, and there was reversal in the thickness of the uterine lining.

About 6 months ago (around 2 years post-Mirena insertion), periods really re-started (more than spotting). Lots of clotting, but not lasting longer than 5 days or so. However, in the last couple of months, the bleeding seems to last longer, more clotting. I'm currently on Day 8 of bleeding (mostly clotted blood). I'm worrying that I'm back to the uterine lining having built up again . . .

Anyone experience this? My original gynecologist wanted to jump to a hysterectomy. I got referred to a different gyno by my oncologist. New gyno recommended the Mirena route. If the bleeding keeps going, of course, I'll go back to my gyno . . . ut I'm hoping there are more options beyond stopping Tamoxifen or having a hysterectomy?
