Who should manage care of breast "issues"?

I am 36 years old and 17 weeks pregnant with my third baby and recently developed a red patch (about 2 inches by 3 inches) on my left breast. It does not itch and is not hot. This breast had a biopsy and imaging about 6 months ago for a palpable lump that pathology reported as chronic infection, mastitis (was not breastfeeding at the time). A little history, my mom, aunt, and maternal grandmother had breast cancer in their late 50's and family carries the PALB2 gene which has a risk profile similar to BRCA2.I do not know my genetic status yet. Anyway, I am wondering which doctors I should have in my entourage to help me navigate the current problem with my breast and my anxiety that I have inflammatory breast cancer! My OB started me today on a week of antibiotics. I want to have next steps lined up for this diagnostic process since it can take so long to get in with doctors. If the antibiotics do nothing, what is a reasonable next step in diagnosing this issue? Dermatology for skin biopsy? Breast surgeon? Thanks for your responses.

    i would say derm next if it's on the skin and doesn't resolve. If you want to get in faster, maybe see if one of your doctors can get you in behind the scenes. That's how I got a next day derm appointment next.
