Mastectomy vs lumpectomy

Hello all,

I was diagnosed with ER+ PR+ breast cancer when I was 30 years old. I had a single mastectomy, chemo and radiation. BRCA1/2 negative

My sister was recently diagnosed with TNBC -- she is 34 years old. She is also BRCA 1/2 negative. I also had the full panel of genetic testing done recently as 9 years ago I was only tested for BRCA 1 and 2 -- all mutations still negative.

My question is for my sister -- she started the neoadjuvant chemo for her 3cm large tumor with EC and carboplatin and taxol. She didnt have much response to EC and hence only has 3 cycles. The carboplatin has shrunk her tumour to half. Her doctor is leaning towards a lumpectomy. Their reasoning is that TNBC usually has chances of re-occuring in other areas rather than the breast itself or could recur in the scar tissue(, hence lumpectomy would be equally effective as a mastectomy.

My sister is torn between deciding whether she should opt for bilateral mastectomy vs lumpectomy. Also given the fact that both of us were diagnosed at such a young stage even though we dont have any "KNOWN" genetic mutation. Bilateral mastectomy is much more aggressive but yet at the same time it may/ maynot reduce the chance of another recurrence in the other breast.
