Intermediate Oncotype and Low Mamma Print--Chemo Dilemma

I received an intermediate score of 23 on the Oncotype. I was surprised, as was my oncologist. He suggested chemo was my choice (suggested 4 rounds of TC). 6mm, clean margins, no lymph node involvement. ER+ (90%) /PR+ (99%) HER2- Stage 1a, Grade 1 "5% proliferative activity" (is this the same is a Ki67 score?

I suggested the Mamma Print--thanks to the all of the great responses on this board! My results came back as a low risk. Meeting for a 2nd opinion tomorrow to discuss.

I had resigned myself to the fact I would do chemo, radiation, and Tamox to decrease future recurrence chances from 15% to 11%. But now that I know the MammaPrint score is low I am not so sure. I want to do all I can to increase my chances of this being a "one and done" but also don't want to do chemo if it won't be that beneficial. I am 45 years old and in otherwise great health.

Anyone with similar results have any thoughts?

So new to this and still trying to wrap my brain around it.

    Are you kidding me? Grade 1 strongly er and pr positive with an intermediate oncodx score no way would I do chemo.

    I had a low risk Mammaprint, with a larger tumor than yours, one positive node, and a high Ki67, and they strongly encouraged me to not do chemo. I went right to radiation instead.

    My second b.c. was 7mm, grade 3 and I didn't get ki67 nor oncotype....the oncologist said I was low risk. With what you have, I think it is low risk as well. Tumours that are 1cm or less are usually low risk except for Her+ and triple negative OR any with positive nodes. Your high HR+ which means anti-hormonal will work REALLY well as systemic treatment.

    I am not sure if I am low risk or not because I never got Oncotype or Mammaprint done, but my tumor did have a lot of "low risk" features - highly hormone positive (almost ER PR both 100%) and low Ki67 (tumor 8%, positive lymph node 7.5%) , except for it was grade 3. I actually have received the heaviest treatment - 12 rounds of chemo, mastectomy, 33 rounds of radiation, OFS + Aromasin, and even 6 rounds of xeloda for prevention. I am glad I have done everything I can, no regret!

    Good morning. My stats are similar except that I am 59 and my Oncotype score was 18. My MO said no way would she even consider chemo for me unless my score was 25 and even then it was a gray area. So I would personally vote against chemo but urge you to mentally commit to five years or more of Tamoxifen or an AI. It will kick ass for you given high ER scores.

    My tumors were larger than yours, but my Oncotypes were 16 and 20 (I didn't do Mammaprint). My onc wouldn't even let me discuss the idea of chemo. He pleaded with me, "I've killed people by giving chemo." While he initially wanted me to do 10 years of Tamoxifen, in order to appease my "but I want to do everything" mentality, he offered me Zoladex (to shut down my ovaries and drop my body's estrogen levels), following with an AI once I prove to be in menopause (Tamoxifen in the meantime).

    Such a personal decision, and I do wonder sometimes, "should I have done the chemo?" But this is the path I chose.
