Questions about a Mixed diagnoses
Hello- just got back a nice low onco. score of 7 last week and after celebrating no chemo, a little voice in my head said- What about the other lump? My diagnosis is ILC and that seems to be the driver here in the testing and treatment plan. Meanwhile the other lump was a little satellite tumor of Invasive Tubular carcinoma that has not been mentioned. I understand that these are slow growing, better outcome, etc... but, is it being addressed? Should I have had an oncotype test for it as well? I will see the doctor next week but may have to call before- driving me crazy.

I had multi-focal ILC - three spots. They sent all three for onctotypeDX but one of the three couldn't be tested (not enough tumor?) so I got two scores.

Hi there Leigh. I had one mixed IDC/ILC tumor and one smaller ILC; lumpectomy; radiation; an Oncotype DX score of 18; two opinions; and no chemo. Was the second little spot also removed during your surgery? Assuming it was, either Tamoxifen or an AI will provide excellent treatment to prevent recurrence no matter how many tumors you had or what kind. Happy to provide more info. if helpful since my MO and I had a good talk about it. But it sounds like you've got a great plan going.

Sorry to hijack thread, but I had s question for Georgia. There are so few of us with two tumor types found, and so little info on this scenario, especially if you are premenopausal as well
Georgia, I’ve read different thing about Tamoxifens efficacy for ILC. Did your MO discuss any if that? I’m premenopausal, so tamoxifen was what was recommended

Hi there Chiarara. I have osteoporosis, so even tho I'm 59 and postmenopausal we decided to do Tamoxifen, at least for two years. Then we might discuss switching to an AI if my bones are in better shape, which they might be with the Tamoxifen + Fosomax. The conversation my MO and I had was fairly brief, but as you know ILC has a slight tendency to reoccur in the opposite breast and her main thing was that I really needed to take an anti-hormonal of some kind. She also noted that AIs and Tamoxifen are about the same in terms of reducing recurrence in the first ten years; it's after that when an AI shows slightly better results.
So that's a long way of saying no, I didn't get any particularly specific findings on ILC or mixed histology vs IDC; there may not be much out there. However, she implied Tamoxifen works the same on all types as long as they're ER+. Thank goodness the science is getting better every day so I'll ask her again next time I see her.

Did you have a lumpectomy on both or a mastectomy? I would think the treatment would be the same if it was a very small tumor. Was it noted on pathology the grade of the little tumor?
I asked the same about Tamoxifen on ILC, and my surgeon and oncologist both said it is the hormonal factor that it is working at. Keeping or reducing the amount of estrogen that could bind to any leftover cells.

I had a lumpectomy and sentinel node excision. ILC tumor was 3.1cm and IDC Tubular tumor was 1cm. Nodes were clear. Hormone positive, HER2-neg., post-menopausal, 60 yrs. old. Thx!
