Revisions and Insurance Coverage

I know that in the US, insurance companies are required to cover reconstructive surgeries for BC patients but I wondered about revisions of those same reconstructive surgeries. For example, for many women who need or choose to have revisions (ie., change to different implant, need a 2nd round of fat grafting, etc.), is insurance required to cover those as well? I understand the typical deductible and max out of pocket expenses are paid by the patient but costs related to revisions beyond that is what I'm curious about.

    If your initial surgery was covered, insurance will also cover revisions of the original surgery.

    I had to have revisions, even without recon, as I was left with dog ears and extra fat that made me look lumpy and weird and insurance covered the revision (save for my copay).

    It's worth double checking your specific plan just to make sure but, in the US, I'm reasonably sure they have to cover revisions if the original surgery was covered.
