Please Help me with Birads 4 Report- core biopsy already done.
Hello. I am back AGAIN. I had DCIS in 2008, grade 2- lumpectomy and 7 weeks radiation. No tamoxifen. Right breast only. Age 37.
My symptoms Prior to finding the lump: breast pain in upper quadrants of right breast- not bad pain- I was thinking it was hormone related. Bee stinging sensation that would come and go , a few times a day but would go away quickly..
I Found the lump myself 6 weeks ago. Had mammogram and ultrasound - shows 2cm mass at 1:00 position in chest wall- feels like it's sitting on top of my muscle- but I don't know. Prior to finding the lump, i had my yearly mammogram in March- was clear- had an ultrasound end of May- with a birads 2 score. October 30th I had ultrasound guided core needle biopsy- expect results soon. The radiologst verbally told me on 10/27 it looks malignant, suggested MRI and petscan. And was concerned about a node in my axilla. And said I cannot have radiation again- And would be looking at a mastectomy. After saying all this - he said it COULD be benign...but has characteristics of malignant. Irregular border, round not oval, and sees vascularity , I think he said three lines to the tumor, ( not sure on that as my head was spinning and I was in tears)and image is dark on screen. I asked him my Birads score he would be putting on my report and he stated a Birads 4/5. My report reads a 4.
REPORT reads: Impression - suspicious right axillary tail 2 cm palpable mass which needs ultrasound guided biopsy. ACR Birads 4 ( suspicious abnormality)
FINDINGS: mammogram: scattered fibroglandular tissues. The spot views show a lobulated mass in the right axillary tail.
BREAST ultrasound : The targeted right breast ultrasound shows a 2 cm irregular hypoechoic mass in the axillary tail corresponding to a palpable lump.
A friend of mine who is a retired nurse says it feels a bit "squishy and slightly movable " to me it doesn't move and feels hard... so I don't know
I am so worried for my results. I'm concerned that nothing was showing on my previous imaging because to me it's rather high up on the breast .. I'm wondering could this be a reoccurrence to the chest wall? -I've seen a person on this board who responds to these reports as a radiologist I believe. I am hoping for some input from him as well as anyone else who has been through this with Birads 4. Thank you

I am so sorry that you're here, concerned about a recurrence. Your stats are similar to mine, and you're reporting, of course, the thing I dread after 14/15 years NED. You're referring to DJMammo. I'll send him a PM and see if he can lend his expertise to your report deciphering. I'll not add my two cents, but just send you big ((hugs)) and hopes that your doc is wrong.

The description of the mass is worrisome as are the radiologist’s comments to you after your scan. Add to that your history and the odds of this being benign decrease.
Let us know what the biopsy shows. Afterwards you may need additional imaging such as an MRI and perhaps a PET scan if your nodes are positive. Good luck.

Thank you for your responses. I'm still waiting for test results. I called my Dr. Office yesterday and they don't have anything yet.

Unfortunately no. I have an appointment with my PCP this Thursday that was already booked prior to all of this. Waiting is making me sick with fear and anxiety.

I find out my biopsy results this afternoon. Results are in. I will come back and post them. I'm not ready for a BC diagnosis again.

Invasive mammary carcinoma medullary features, provisional mBR GRADE 2
Tumor type invasive mammary carcinoma. Modified bloom and Richardson classification ( m,BR score 7/9
Tubule formation 3/3
Nuclear grade 3/3
Mitotic activity 1/3
Invasive tumor size ( pathologic) 0.5 cm
Tumor present in 2 of 2 submitted cores.
Vascular invasion - not identified.invasive tumor necrosis: present
Breast prognostic panel :
Estrogen receptor (,ER ) Positive 1-3% stain intensity weak
Progesterone receptor (PR) Absent / Negative 0%
HER2 (IHC) not over expressed (1+) EMI
A referral put in today to a surgeon, should get appointment next week. My doc said surgeon will want to remove the tumor and test lymph nodes in armpit. Then go from there.
Can anyone help me on the breaking down of my report? I'm trying to stay off DR. Google!
I'm devastated to be here again. First cancer was 3/08 DCIS , stage 0 lumpectomy and 37 radiation rounds
My PCP doc wasn't able to tell me if this is a reoccurrence or a new cancer. That info will come in a later pathology report?

This is an amazing place to be for help. Both in the tears and the smiles. Is this in the same breast you had a lumpectomy in? Never mind, looks like it is. Did you have genetic testing back in 08? Not sure if was even available then though.
Might be something to check in to as you go forward.

I met with a genetics counselor back in 2008. I didn't "qualify" back then because I was told I was young and my cancer and my mothers cancers were not invasive. My mom had DCIS and pagets- she had a lumpectomy and removal of the nipple and areola. No chemo or radiation.
I had radiation and lumpectomy right side. New cancer is in same breast

Science has came a long ways in genetics, I would ask again. You never know. And...a long ways in treatment options.
More importantly, have NO REGRETS! You did nothing to cause this, you didn't not "do enough" before. It's a new season, new sience, new treatments. Maybe even consider new doctors if you haven't already. DO NOT look backwards, one day at a time going forward.
Look to the future, find a new action plan, a good team and go to battle. We are fighting with you.

So, so sorry. Seems like the tumor is small, which is good, so focus on that. Your surgeon can help you decide about whether to choose a masectomy, and don't hesitate to get a second opinion. We are here for you.
