Fulvestrant in the Preventative, Adjuvant Setting

    My mother had IIIA ER+/PR-/HER2+ cancer and completed neoadjuvant, surgery, radiation, and adjuvant treatment. She is now in the preventative setting but is having trouble with adherence to AIs because of AIMSS.

    Has anyone used fulvestrant in the preventative setting? One advantage is that the drug is dosed monthly (via injection), so adherence is pretty much guaranteed.

    Also, the recent Lancet study showed significant improvement over anastrozole in the first-line setting for advanced or metastatic cancer. Would these results apply to the preventative setting?

    Lastly, would fulvestrant have long-term side effects that make it inappropriate to use over a 5-10 year period?

    I understand that insurance coverage would be iffy, but we've had luck with coverage before (my dad's insurance company has been great with us).
