Excisional biopsy birads 5
hello, I had a callback on my yearly mammogram, did the diagnostic mammo and ultrasound Monday. The radiologist came in to speak with me told me she was giving it a Birads 5 that I would need to get a surgeon. I don't have my report will get a copy Friday. I am scheduled to see surgeon Friday morning with excision all biopsy Monday. They wanted to do it this week but my workload prevents it. I have had biopsies 5 times the worst thing it ever was was ALH. All of my past biopsies were done core needle with one follow up excisional for the ALH. I'm just wondering why excisional instead of core? Same breast right side where ALH was foundin 2011. Found a lump there in 2014 but nothing seen on mammo or ultrasound. Lump is still there and as the radiologist put it you can see it now.

kmath, I'm not sure how they determine what type of biopsy to do. A good question for your surgeon. Maybe the location and expected size of the lump. A lobular diagnosis is usually harder to detect with mammo and US, than ductal, and may require an MRI. You may want to address that with your radiologist and surgeon as well. You can always get another opinion if you are not getting the answers to your questions. Good luck movng forward.

Got on patient portal last night first mammogram report was there. 1.8 spiculated mass 1:00 position of right breast. Today will be a long day waiting until I get to see surgeon tomorrow. Dread the excisional as my job has me lifting 20 or more pounds all day and we are short handed at work so I don't need to be off long.

Good luck with your surgery tomorrow. I think I was told to not lift over 10 lbs for about a week. Take care of yourself:)

Thank you! The consult it tomorrow the excisional is Monday. My ultrasound report showed up today on the portal.
It states there is a 1.8 round mass with an angular and spiculated margin in the right breast at 1 o'clock middle depth 8.5 cm from the nipple. The round mas is hypoechoic with an echogenic boundary and posterior shadowing.
Impression: Highly suggestive of malignancy- follow up recommended
The 1.8 cm round mass in right breast most likely is carcinoma and is highly suggestive of malignancy. A biopsy and a surgical consult are recommended.
I don't like anything about this report at all! I hope the biopsy results come in quickly and are benign so I can think about something else.

Sorry that you got such a worrisome report. And when you have your biopsy, take the time you need and don't lift more than the surgeon allows.We all have worried about our jobs, but your healing is paramount. It's tough to be shorthanded and very commendable of you to worry about your coworkers, but this is one time you have to put yourself first.

My husband and I met with the surgeon yesterday. No excisional Monday thankful for that will be having core needle Wednesday morning, a longer wait though. Pathology should be back Friday of next week or the following Monday. He went over surgeries, breast cancer types, receptors all kinds of things he gave us literature of what he discussed. He kept saying he wasn't saying I had breast cancer he couldn't without pathology. He said he had had two patients score a birads five with benign pathology so I could to. He was honest though and said he thought mine would be malignant but probably stage one or two we would know more after pathology. He showed us a card with a hole in it showing the size of my mass, I kinda lost it. I was thinking pea not walnut.
I am really angry at myself I skipped last years mammogram due to moving and a new job no insurance yet I just kept putting it off. I wasn't keeping a eye on the lump like I was supposed to wasn't doing self checks at all. The lump was clear in 2014 my 2015 mammo was fine too. But with my family history I should have been doing my checks. If I had I would have noticed the stupid lump had grown! The surgeon had no problem finding it because it had a chance to get bigger because I wasn't checking. I was and am so tired of all the mammograms, ultrasounds biopsies and scares I had been through. They would scare me and I'd wait on results and they would be benign. It was like I was being held hostage by my breasts and family history. But I knew better. I still have hope that this is just another scare but at the same time I am angry and terrified

KM, It sounds like you are moving in the right direction! Don't beat yourself up about the past. It won't help anything moving forward. One day at a time. You can do this:)

thank you both. Having core needle at 10:30 tomorrow then some more waiting. Will be working Thursday Friday and Saturday so will be busy n that will help. It's the evenings that are the worst for waiting I hope to hear results this week waiting through the weekend will be toug

I agree with KeeptheFaith, beating yourself up doesn't help you. Try right now, while you're waiting, to pamper yourself. Go do something that takes your (and your husband's) mind off of what may or may not happen. Deep clean something. Go for a long walk, or swim. Play with your pets, kids/grandkids, friends. Practice relaxation (breathing, mediation, listening to soothing music, guided imagery) as this will serve you well regardless of your final pathology report. Reducing your anxiety and frustration will allow you to move forward with a more clear head. You need that to make good decisions, either way. Sending positive thoughts for B9 results. We're "in your pocket"!

didn't have to do much waiting, my surgeons nurse called this afternoon. The tumor is malignant and invasive. That is all I know right now. She called back about an hour ago and left a message that she had spoke with pathologist again and would call me back. I wasn't expecting a call again or I would have had my phone on me.

Thank you. I did not hear back from the nurse yet so I do not know anything more. It makes my mind wonder I wish I at least knew what kind of breast cancer it is. The surgeon did mention the nodes looked clear on scan, I know they won't know for sure until tested. He also said he thought a lumpectomy would be all I needed again this was subject to change depending on pathology. After so many biopsies I said if ever it is malignant I will choose mastectomy without reconstruction. I have had biopsies on both sides and waited for results each time harder to get through. I am done with it. I am terrified of the thought of surgery and knowing I have cancer. The first time I said I have breast cancer I choked. Anxiety so bad this morning I had to pull over on the side of the road. I am just scared

KM, so sorry to hear the news. It is so over-whelming. If you can't deal with your anxiety and are open to it, ask your Dr for anxiety meds and/or a sleep aid. If you have someone to take to your next appt, I would ask them. Someone you can trust and that can take notes, if needed, etc....as opposed to someone that you will have to "help" deal with it. You have every right to be scared. Trust me when I tell you that you will feel better once you have a better understanding of what you are dealing with and start your treatment. Stay away from Dr Google and try to do something fun to keep your mind from wandering. Wishing you the best. ((HUGS))

Thank you, my surgeon called with my receptors today, I have a appointment with him in the morning as well. It is ER+, PR+ and HER-, grade 2. He had mentioned that he felt he could do a lumpectomy at our first appointment, today he said he would still be open to that or to a mastectomy. Because of my age, family history and excessive scarring from a reduction I had he would do a mastectomy if I wanted to. He said my chances of something being hidden due to the scarring were high. He did say because of my age a mastectomy only on the one side could cause me balance problems or back pain as I age, I am 46. I have thought on it for several years when I had other biopsy scares, I want a mastectomy both sides, no reconstruction at this time. he did tell me I could have reconstruction at another time if I choose to. He said it is possible I would be a canidate for tamoxifen or another pill I cant remember the name of but that it depends on staging after surgery. He said the size of my tumor can change with surgery as well. He still feels like I will be stage 1 or 2 though so thats good.
So I guess I will be moving to another board unfortunately. And fill out some things I learned today.Again thank you all for your help.

Kmath I suggest you check out the mastectomy group. I had a double mastectomy 2 months ago and they were a great help there.
