Can it be extranodal extension?

I had another post-chemo ultrasound on Monday. Just saw the update from the radiologist. She actually found a 4x4mm irregular small mass "in the immediate vicinity of this biopsy-proven metastatic lymph node". I am more confused now. Can it be extranodal extension?
