Nipple biopsy with lidocaine before Mastectomy?

my surgeon wants to do this next week. It would involve slicing the nipple and coring the tissue out to test for cancer to see if he can save the nipples or not. Not sure why he can't just do this during the surgery though he says this way will be more accurate. Anyone had this done before surgery? I'm dreading it. Sounds painful!
