Lat Flap confusion !!

I have tissue expanders in now, after my double mx. Because of a horrible path report (putting me at stage 3c), I have to do radiation on left breast (when we thought I was stage 2b I was planning on opting out of it). This means that for my recon, I will need a tissue flap plus implants to help avoid capsular contracture, tightness, lifting, shrinking, asymmetry, and other issues apparently caused by rads which will show up in the future. I am too thin for a DEIP so the lat flap was suggested by 2 different PS.

I don't mind a scar on my back, but I do NOT want an additional breast scar/incision besides the one I have now (it goes across the breast diagonally where my nipple used to be). So my question is, can a lat flap be done without having another big incision on the foob? Does this procedure have to result in the discolored paddle/football shaped patch? I swear I've seen multiple photos of this type of recon, and some look patchy while others don't. Wondering if there is more than one technique for transferring the tissue.
