Dealing With Medicare

Nine years ago I had breast reconstruction after a lumpectomy and then a second surgery when a recurrence was suspected. All totaled about 1/3 of breast removed. At that time I had Humana insurance and it was approved immediately. Now I need a revision. The breast that had the lumpectomy and radiation is about a small B cup. The other breast is a D cup and sags about 2 inches below the other breast.Now I am on Medicare. Medicare will not do a pre-approval and I am concerned that they wont pay for it because it was a lumpectomy. Has anyone dealt with medicare under circumstances similar to mine?
