No biopsy on BIRAD4?

Do you know if sometimes they don't do a biopsy on a BIRAD4 patient?

The reason I'm asking, long story short: found microcals, went to get a CNB, they couldn't find them (which I find hard to believe they magically disappeared after 3 weeks). Now I'm back to square one. Do they just do the "wait and see" thing in this scenario!? Or would they do a surgical biopsy? I don't see how they could do the latter if they can no longer see the microcals.
