Scar tissue question

Hi,

I had a masectomy, radiation and a tram flap back in 1998/1999. I now have a bone met and two lymph nodes that have lit up on my January pet scan. My pet scan stated there were changes in the tissue on my masectomy side. I have some muscle pain in the chest area and in my pit. I believe one of the nodes they saw is in my arm pit. Does this mean cancer is growing there also? And could the pain be scar tissue? I see my onc next week and get blood work this week. Scared of course. I am only on femara and do not know if it is working or not. I feel fine except for aches and pains.
