Pleomorphic ILC
I am concerned because I have now discovered I have pleomorphic ILC. Path report of biopsy says infiltrating carcinoma with features favouring pleomorphic lobular carcinoma. Apparently pleomorphic is aggressive. I haven't got the histology back yet so don't know what grade. I'm still undecided about what type of surgery to have. Would pleomorphic affect the decision to have BMX or lumpectomy. This my second primary BC. MY CT body scan was clear. Anyone else with pleomorphic features?

MRI can help in determining the size of the tumor so that you and your surgeon have more information. ILC may not show well on imaging and can't always be felt either, so it can be hard to get an accurate size. The size and location will influence your surgical decision.
I was dx with PILC, too, over 10 years ago. I went with BMX because I had multiple small tumors in the left breast, and lots of precancerous stuff in the right. My BC showed up well with MRI. We're all unique.
I'm sorry you are dealing with this again. Very good news that the CT was clear.

Yes, I have Pleomorphic ILC but I am triple negative so I don't how that compares to your case. I also didn't know any of this until after I had a lumpectomy and I was told that the lumpectomy and radiotherapy is just as effective as getting a mastectomy, however, if I got a recurrence in the same breast then I think that second time around I would get a mastectomy, but I probably would have no choice by then. What has your Surgeon recommended ?

Hi I have PILC too, second diagnosis in left breast. Mine was not pleomorphic the first time. As I was ER+ PR+ my onc ordered an Oncotype DX to see if chemo would benefit me. Result was 22, lowish Intermediate score. She said hormonal therapy would be of more benefit to me than chemo. I'm now considering prophylactic mastectomy for right breast. I had lumpectomy 12 years ago but it returned so in hindsight I probably would have chosen mastectomy then, but we can't see into the future!
Numb, have you considered asking your doctors to recheck your pathology as my understanding is that ILC is always ER+? Just a thought, I may be wrong.

Thank you Optimist for your reply, and it is good that you are ER+ and can take hormone treatment instead of Chemo.
I was diagnosed as ILC at first, then this was changed to IDC and now I am back to PILC based on an ecadherin test that says that IDC always has some ecadherin and mine is negative for it so I must be ILC. I have made an appt with my Surgeon for next Monday as I am totally confused at this stage and cannot find anyone who is in the same category as me that I can even confab with. Wishing you well with your decision. I feel like having a double mastectomy at this stage I am so worried.
