Triple Positive ILC???

I just got the final pathology report from my BMX. My equivocal Her2+ IHC result was found to be positive after FISH. I am 95% Er+and Pr+.

I felt like I had wrapped my head around the hormonal treatment I knew I would need, was just waiting for results on oncotype test to make chemo decision (3cm tumor,O nodes) but now I don't qualify for the oncotype.

I am shocked. Should I have done chemo before surgery? Is there anybody else out there with this combo? And I am premenopausal to boot, having period now and feeling overwhelmed by this change.

Er and Pr positive 95 percent is good, and her2 positive you can get herceptin. I wonder if your oncologist will recommend hormone therapy and herceptin. Good luck.

Hi. What is your detailed subtype of ILC? Classic, acinar, histiocytoid, solid or pleomorphic? Three last subtypes often show Luminal B-like profile or HER-2 enriched. Im my opinion you should receive double mastectomy, and then adjuvant chemo, trastuzumab and endocrine therapy (LH-RH agonists + aromatase inhibitors).

I'm sorry I wanted to help... But i've noticed recently that many oncologist (especially in US, but more and more often in my country) has a habit of giving chemo before surgery, regardless of tumor size and lymph nodes involvment. I always thought that neoadjuvant systemic treatment is indicated when cancer is diagnosed with primarily inoperable stage (some cases IIIA and IIIC and all cases IIIB) or cancer which primarily required mastectomy to enable BCT...

"So I guess I'm saying that had you said something like "in my country, a patient with your stats might receive xxxxxxx" rather than "IMO you should receive xxxxx" it would be considered more acceptable. Does that make sense?"

    I agree. You're absolutely right
