Switching Insurance Company--still in treatment

Our insurance plan has been discontinued. I as scheduled for two more Herceptin treatments in 2016. If we switch to an HMO, that requires a referral to a specialist, am I going to have to see my primary doctor and get this? Or if we end up with another PPO, it will be a totally different carrier, and I'm wondering if I will need some kind of pre-authorization. Any one have any experience with this? Thanks!! --Lorie
