Reconstruction and weight loss.

I need a mastectomy and would like to have reconstruction. Preferably immediately following -- this seems to have been going on forever, I want to get it over and done with. However, I need to loose weight. The 40 pounds I put on in the last year has made my blood pressure go out of control. When I get back down to my ideal weight my bust will go from it's current 40DD to 36C. The doctor wants to do the surgery within two weeks. No way I can loose 40 pounds in two weeks without wrecking my health. What is the best solution?
