Billing for chemo all over the place

I have had exactly 3 AC treatments in the last 6 weeks. The first billed $230. The second $21286 and the third billed $4k and change. They did the exact same things every single time. I don't really care until I get the final bill but it is making me wonder why. I don't even know whom to ask.
