Why do I even have short term disability insurance???
When I had my BMX back in July, my plan was to only be out for 4 weeks, and my STD posicly doens;t kick in until I have been out for 4 weeks, so I wasn't going to bother trying to file anything. I ended up being out 6 weeks due to an infection, hospital stay and a second surgery, so i got the info on how to file for the two weeks I was out. They wanted me to do up a list of every single doctor appointment, every diagnosis and every prescription medication I took for the last three years (July 2011 through July 2014), not matter what it was for! Up until my BC diagnosis, I didn't go to the doc very often, only if I got sick, so I didn't keep those records. So how the heck am I supposed to know all that?
I could contact all the practices I have ever been to and ask them if I had visits in the last three years and check all the pharmacies to see if I filled prescriptions and see if they have records back that far, but all that work for a lousy 2 weeks' payments (max $500 because the policy is only for $1,000 per month) didn't seem worth the effort. I believe I can print out all my EOBs for 2013 from my insurance website, and I already have everything for 2014 for my taxes, so if it ever comes up a need to file again starting in 2016 I will have everything.
What I don't understand is, if I was wanting to file for a BMX, why do they need info for the time I went to the doc for a sinus infection three years ago? What does that have to do w/ a BC disablility claim? Crazy!!!

I second that Kayb! Experienced the same thing with LTD. They would call weekly for updates and send PAGES of stuff to complete quarterly! I was actually relieved when I got off of it!

Straitlover, they really do not expect you to recall every MD appointment and test and Rx. Give them the big picture:
Rx name and dosage, MD name & location, date used (spring 2008 thru present), diagnosis
I happen to be an insurance agent and all our forms ask for this. They need to know which physicians you have used so they can contact them for your records. The docs then send the info requested and receive $$$ for each page faxed.

Sorry, just saw this response.
That's not what the paperwork I received said. It said to list every doctor, every appointment, diagnosis and medication for the last three years. I don't have it with me right now to give the exact verbage. I will call them and ask for clarification. Don't know if I can file at this late date, though; my surgery was 7/30/14 and I was out of work through mid September.
Also, I know I have nothing for medications since until my surgery, the only RX would have been something for a sinus or bladder infection. I don't always use the same pharmacy, just whichever's closest to where I am at the time, so I couldn't even find out easily if I had anything filled.
