DCIS and size of tumor

I am curious. In doing some research and reading info on this site, I wondered how my 4.2 tumor could be contained DCIS? Earlier mammos and ultrasounds showed a small minute mass. When pathology came back, it indicated I had a 4.2 cm mass without clear margins. How can this size tumor be contained in the small duct? Also, without clear margins, how can they determine the actual size? How could the original size (based on mammos and ultrasound) be "very small" yet turn out to be as big as a walnut? I could ask my surgeon but I won't see her till the day of my next surgery (3/24). Thanks
