Had exchange back in April...lots of rippling

Hi, all. It's been a while since I've been on here so let me bring you up to speed...

My wife had a PBM NS back in November. We went back and forth about "how big was big enough/too big" and just let the expanders expand until we liked the size (we kept liking each new expansion). After a few months of expansion, she exchanged to a 500cc Allergan 45 in April.

From almost the beginning, one side started rippling. It wasn't a major issue, just a little on the outside edge when she would bend a certain way. Plus, we really loved her new shape. She was always on the small side so we enjoyed her new assets. However, as the months have gone on (and I assume the swelling has continued to go down), the rippling has gotten worse. She now has rippling in both breasts, and the rippling on the first breast is starting to stretch across into the center of the mound, into the nipple area.

At what point should we talk to the PS? How much rippling is acceptable? What are our options? Should we consider fat grafting? A different type of implant (Gummy?)
