Thank you for posting this.  I want to run it off and take the time to read it later, but the one thing that jumped out at me already is the potential difference in survival depending on where you are treated -- something I firmly believe based on my own experience of using both a local and a UCLA oncologist -- whom I both love for different reasons, but am also keenly aware of the differences in approaches, including a subtle yet discernible expectation re. survival.   Deanna
