Why would I not have received a Her2 status and oncotype score?

I was dx in April 2005-stage 1 (first tumor was 1.8cm and second, same breast, was smaller). I'm in a big city, dx by a large/well-known breast dx center (they did nothing else).  I assume I got excellent care.Sent to a surgeon who did a phenomenal job, then an onc that I did not care for.  WHO would have normally ordered tests for Her2? 

I know that, at the time, when I noticed others had received an oncotype score, I think it was not being covered by insurance so perhaps not being tested as often, because I was never asked about opting to pay for it. 

But I'm also curious about Her2.  I never got that info, and I did have a copy of my reports (though, after 3 moves, I've lost them-geesh!). I know I'd have noticed it, because I scoured those reports.  I know my grade (and the individual grades within it) and stage and ER/PR status (+/+).  But I never got a Her2 status.  Was 2005 too early for that?  Would it have been something the surgeon ordered when my tumors were sent to pathology?  I've read some things on here that a Her2 positive status means an oncotype score is not relevant (or am I backwards on that?).  I wasn't really given an option re: chemo.  It was prescribed, but...long story made short...I took one treatment and ended up stopping it. Is a chemo decision the only real reason to have wanted those pathological data? Just curious if anyone has an opinion.

    I am wondering if the Her2 testing was done independently of other pathology and the report from that is on an amendment to your regular path report, and you were never given a copy.  It is common for Her2 testing to be done as a send out, not by the same lab that does your other testing, and it is often delayed and commonly reported separately.  As far as Herceptin, it was not approved for use by early stage breast cancer patients in '05, so maybe that is why your Her2 status was not discussed?  Oncotype Dx was a fairly new test at the time of your diagnosis, so maybe your onc was not using it, or insurance was not widely covering it?   Oncotype Dx is not used for Her2+, only for ER+, Her2-, per the website's info.
