DCIS with Micro Invasive...
Hi Folks,
I was recently diagnosed with DCIS with Micro Invasive, 1 Lymph Node (+) 0.6mm, HER2 (-), ER (+), PR(+), I am 46 year old, I has lumpectomy 8 weeks ago...I wonder what is the proper treatment for my case as mentioned on the topic? I am very confused now. Is there anyone who has the same as mine? Please kindly advise your suggestion on treatment.

Summerhappy, with micromets to your node, you are Stage IB. That nodal involvement puts you in a different position than someone who has pure DCIS, or even someone who has DCIS-Mi with no nodal involvement (that was my diagnosis). The most significant and potentially concerning part of your diagnosis is the nodal micromets; what may change your treatment plan more than just the microinvasion alone.
Here is information on staging: Stage Information for Breast Cancer . You can see that with DCIS-Mi with nodal micromets, your TNM status is T1mi/N1mi/M0. That's Stage IB.
And here is a link to the NCCN Treatment Guidelines for Patients - Breast Cancer. The NCCN guidelines are used by most cancer specialists in the U.S.. There is also a physician's version of the guidelines that provides much more detail, but you need to register and sign in to be able to view that document.
Your diagnosis is covered on page 71 of the Patient's Guidelines. For someone who has a microinvasive breast tumor and lymph nodal involvement of <2mm, it indicates "Possible chemotherapy". In the notes at the bottom of the page, it says "For the smallest of these breast tumors with no cancer cells found in the lymph nodes, no adjuvant treatment is needed. For breast tumors of the same size with tiny lymph node tumors, chemotherapy may be helpful"
So that kind of leaves it up in the air, with no definitive recommendation. To find out what others have done, I'd suggest that you repost your question in the Stage I Breast Cancer Forum, perhaps under a title such as "Treatment for microinvasive cancer with micromets to node?" -

Hi there - I had IDC and my tumor was .6cm with micro mets in 1 lymphnode. I got three opinions and all three doctors insisted on chemotherapy (especially given my age of 43).
