The false-negative rate with sentinel node biopsy (SLN) exceeded the expected rate in women with clinically node-positive breast cancer after neoadjuvant chemotherapy, results of a multicenter clinical trial showed.

    interesting...this is why I asked for level 1 nodes to be radiated even though I was clinically node negative before neoadjuvant and my SN were clean. I was concerned about the false negative rate as well

    This was back in 2007, but my surgeon was very leery about the accuracy of the SLN. I had a big tumor that came out of nowhere (6 months after a clear mammo) and he insisted on a traditional axillary lymph node dissection. Even though it came back negative, I'm kind of glad that he did; as we knew, for sure, that the nodes were clear.

    My SNB revealed 20 IST, so I would have been declared node negative by today's standards. I had no nodes stand out on any imaging used pre-surgically (mammo, US and MRI) but my BS (who was a pioneer of SNB) and MO insisted on ALND because I was Her2+. That surgery revealed a much larger node up the chain, which is unusual.
