IDC Stage 2A ER+ grade 3 with Ki67 of 64% and extensive LVI.
Jan 2013 I went for my yearly mammogram. Called back in early Feb for extra films and ultrasound. Due to menopause as well and changes said come back and lets check again in August. August 15th more films and ultra sound with biopsy the next day. Two tumors 2.1 9 oclock benign and 1.7 at 9:30 IDC. MRI revealed 2 additional spots at 12 and 2 at 7. MRI biopsy shows all 4 benign. Lumpectomy done on 9.12 and received the following pathology today at my surgical follow up:
IDC Stage 2A ER+ grade 3 with Ki67 of 64% and extensive LVI. Tumors were 2.1 and 1.7 with positive invasive carcinoma superior lateral margins away from nipple after lumpectomy.
Re-excision is a must, however due to LVI, grade 3 and Ki67 being high wondering should I consider mastectomy this tiime?
Surgeon wants to wait 3 weeks to redo surgery and to see oncologist next week to see if chemo needed and should it be done before surgery.
Scared as I want the cancer out, yet want it treated to be sure the LVI has not allowed it to move anywhere else in my body.

I can't answer for your situation. I liked doing chemo first to start working on anything that may e circulating. I did BMX and radiation on the affected side. -
Have you consulted an oncologist too? I found that while I loved my surgeon, I wished that I had consulted him prior to my mastectomy - I should have had neoadjuvent due to tumor size but since I only went to surgeon and then went to onc for first appt after surgery- I really didn't have all the expert opinions to make the best treatment decisions
But. With all that said.... I am doing fine and in the end it may or may not have made a difference. Btw I had areas of lvi that was extensive as well and extra capsular extension -- I had very low ki67 however 9% - I can' t imagine how ? It's for sure very scary but definitely treatable and not the end of the world. I just tell myself that surgery got it and chemo mopped the rest up and radiation and now my ai won't the devil come back. It will be the same for you whatever you decide to do -
If those tumors are in different quadrants, mastectomy would actually be necessary. I would strongly advise chemo. You have a grade 3 with extensive LVI and that has to be addressed. Many women get chemo before surgery to shrink tumors for a lumpectomy...you might want to ask about this if your tumors are in same quadreant. I would. Anti hormonals will be your next best friend and would advise them for 10 years (I have to have them for that long and they have great benefit in preventing distant mets). I wouldn't even do the oncotype test either. Extensive LVI, grade 3 and high Ki67 pretty much indicates chemo. But don't get alarmed; you are still early stage and very treatable...and aggressive cells respond well to chemo. Best wishes, hang in there! ((((hugs))))
