insurance for rt.delayed masrectomy

I have a couple posts in other topics about this subject. Just want to see if anyone has done this and if insurance will go for it, or do I need brest surgeon and oncologist permission too? Had left mastectomy 4 years ago. Didnt have enough info or a place like this. I regret not having a bilateral, now that I have a plastic surgeon in my town, I want other brest removed and reconstruction. Has anyone gone through this and how did it go with insurance. Did they approve or did you have to fight?
