Stephanie, a friend of mine on another forum who conducts extensive research, kindly provided the link below and asked that I share it.  It's especially helpful in that it is global, not merely US-focused.  This list of international resources includes general resources, palliative care and hospice directories, advance care planning, caregiver resources, etc.

Hopefully most readers won't need to refer to it for years and years, and the link can be saved for reference.
