Lower back is killing me. Can I put hot water bottle on lower back(medius area) or is this lower back area considered part of the quadrant section to avoid? I guess if heat is a no no then ice would be too.

We're probably all a bit different, but I have truncal LE, and I don't hesitate to use a microwaved heating pad on a lower extremity or on my lower abdomen or lower back.  The no heat rule applies to the area that is prone to swell -- in my case, my side (the worst area), upper back and upper arm.

As far as ice goes, I would think that would also be okay on your lower back, but not on any area that's painful due to LE swelling. From what I've learned here, ice or cold water might feel good on an LE flare, but the long term effect of the cold is detrimental.   Deanna
