Had NSM & new Mammo shows microcal under saved nipple

I had lumpectomy 3 years ago in September 2009 on my left breast. It was Comedo type triple negative DCIS.

After one year, mammo showed microcalcifications. The stereo biopsy confirmed exact same type DCIS. It could be left over from the previous lumpectomy. So I had the Nipple Sparing Mastectomy in October 2010.

Last week annual mammo showed microcalcifications under the saved nipple area which is the only breast tissues I have after NSM. The radiologist told me that he can't do the needle biopsy since I don't have enough breast tissues to have the needle biopsy and suggested to see the surgeon who performed the MSM for the surgical biopsy.

I had an appointment with the surgeon last Friday, 09/14/12. He told me that he had to take out the remaining nipple and areola area to do the biopsy. And I asked whether he could do biopsy first before removing everything. But he told he had to remve everything to do the biopsy.

I am wondering whether the biopsy can be performed without removing everything leftover from NSM or not.
