Anyone go to bigger implants? Does insurance cover switch?

    I am 3 months out from my exchange surgery. Now that the swelling is down and the nipples are completed,  I feel like they are too small. I have Allergan Naturelle high profile round silicone 425ccs, weigh 130 and am 5'4. My PS told me initially that I cant go bigger because of my frame and that going bigger makes the implant too wide.  I find this hard to believe. Seems many on these boards are smaller than me w/bigger implants.  What are my options? Will insurance cover another surgery? Does it matter how far I was expanded as to whether they can put in bigger implants? Does fat grafting make sense to make them look larger? Finally does going bigger mess up the placement of the nipple? Ugh... any advice is greatly appreciated.
