Pathology after surgery

Scheduled for lumpectomy/ axillary node dissection.  First path report from core biopsy came back Her2+

Does it make sense to confirm that when the path from the surgery is sent out that all 4 tests for HER2 are performed (IHC, FISH, SPot and Inform HER2 Dual ISH).  Does the BS have to request this or should I make sure this is done?

    It is standard to run IHC and if the results are equivocal to confirm with FISH.  Running all the tests will not change anything.  Besides with a Her2+ core biopsy, you are likely to get herceptin regardless of the results after surgery.  Tumors can vary and they only test a small portion--therefore it is not very unusual to have a bipsy positive and a tumor negative or vice versa.  I had this situation. My onc said that if there is any evidence of Her2+ they treat with herceptin.  I hope this helps!
