end of life coverage

I found out last night that my fiance's health care plan, which will cover me for any treatment I need, doesn't cover any "end of life" care. No in-home care, etc. It covers hospice, but there are limited beds available and it's in a city 60 miles from our home. The numbers he got were that out of pocket, in-home care would likely run about $5-6K per month.

I'm wondering if this is normal or how other people deal with this stuff. I know that there is long-term insurance, but with already having a stage IV dx, I'd guess it's a bit late to get onboard with that.

What do people do when it comes to end of life care? Do our families all just end up bankrupt when we're gone? Just trying to figure out some options, so any suggestions or information you want to provide would be appreciated.
